Value-based digital health: A systematic literature review of the value elements of digital health care.

Objective: To carry out a systematic literature review to identify the Value Based Digital Health elements.

Methods: The search for records was carried out for the period 2012 to January 2024, in two main databases, EMBASE and Scopus. Records were limited to studies published since 2012.

Organizational readiness for knowledge translation in chronic care: a Delphi study.

Background: Health-care organizations need to be ready prior to implement evidence-based interventions. In this study, we sought to achieve consensus on a framework to assess the readiness of health-care organizations to implement evidence-based interventions in the context of chronic care.

Methods: We conducted a web-based modified Delphi study between March and May 2013.

What older complex chronic patients need to know about their everyday medication for safe drug use.

Objective: To determine whether elderly patients with complex drug regimens have enough information to ensure safe drug use.

Methods: A cross-sectional study was conducted in Spain based on interviews of a random sample of 265 patients aged over 65 and taking at least five drugs for multiple comorbidities. General practitioners reported on diagnoses, drugs, dosage and biochemical laboratory results.

Because of the steady increase in healthcare complexity, due to high specialization and the involvement of a number of services, as well as the increase in patients with chronic diseases and pluripathology, coordination has become a high-priority need in healthcare systems. The distinct regional services that comprise the decentralized Spanish National Health System have developed a number of experiences to improve collaboration among the providers involved in the healthcare process. The present article aims to analyze the experiences with integrated healthcare providers in Catalonia and the Basque Country and the chronic diseases programs of the latter.

Integrated care for chronic conditions: the contribution of the ICCC Framework.

Objective: The aim of this research is to highlight the current relevance of the Innovative Care for Chronic Conditions (ICCC) Framework, as a model for change in health systems towards better care for chronic conditions, as well as to assess its impact on health policy development and healthcare redesign to date.

Methods: The authors reviewed the literature to identify initiatives designed and implemented following the ICCC Framework. They also reviewed the evidence on the effectiveness, cost-effectiveness and feasibility of the ICCC and the earlier Chronic Care Model (CCM) that inspired it.
