The aim of the study was to analyze the prognostic implications of 3 myocardial necrosis markers measured at admission in short-term observation of patients with suspected acute coronary syndrome. The study group consisted of 336 consecutive patients whose concentration of cardiac troponin I, creatine kinase-MB fraction, and myoglobin were measured at admission. We describe the case of a 51-year-old female with previously undiagnosed congenitally corrected transposition of the great arteries (ccTGA) who was admitted to the hospital due to the exacerbated symptoms of heart failure for planned coronary angiography. The aim of the project was to evaluate several of the most interesting Intemet "interactive patient" programs, as well as simulations of real clinical cases, and to compare them with a program "The Simulation of Emergency Cases" described in this work, and created by the Students' Research Group at the Department of Medical Informatics, the Medical University of Warsaw. The aim of the project was to create a computer program--expert system, which will support a doctor when a management for patients with acute coronary syndrome needs to be chosen. The expert system consists of four modules: knowledge base, previous cases database, inference engine and explanation module. Knowledge base was created with support of clinical experts, based on current management standards, guidelines and results of clinical trials according to evidence-based medicine rules.
