Objective: To develop evidence-based local clinical practice guidelines and primary care referral pathways for general physicians to help streamline the management of glomerular diseases and diabetes in chronic kidney disease patients in Pakistan.

Methods: The study was conducted from October 2021 to February 2023 at the Centre for Clinical Best Practices, Aga Khan University Hospital, Karachi, in collaboration with the AKUH Section of Nephrology. Two source guidelines were selected by the local nephrologists after a thorough literature review on PubMed and Google Scholar.

Introduction: Neoadjuvant chemoimmunotherapy has achieved overall survival (OS) benefit for patients with resectable non-small cell lung cancer (NSCLC). Here, we present outcomes after 3 years of follow-up from the first reported study of neoadjuvant atezolizumab+chemotherapy.

Methods: This open-label, multicenter single-arm investigator-initiated phase II study conducted at three US hospitals tested up to four cycles of atezolizumab, carboplatin, and nab-paclitaxel prior to surgery.

Introduction Anthracosis is black discoloration of the bronchi, which can sometimes cause anthracofibrosis. Usually, exposure to biomass fuel, air pollution, or smoke at the workplace causes it. The objective of the current study was to determine the association between tuberculosis and anthracosis.

  • Acute pseudo-obstruction of the colon, known as Ogilvie's syndrome, is a rare complication that can occur after surgery, most commonly seen after cesarean sections, with only one previous case linked to vaginal delivery.
  • A case report details a 31-year-old woman who experienced severe abdominal pain and bowel perforation a week after giving birth vaginally, necessitating surgical intervention.
  • The report highlights the importance of early detection and treatment of this condition to reduce the risk of serious outcomes, including death.

Article Synopsis
  • The study investigates the effects of heterologous (different) and homologous (same) COVID-19 vaccine booster doses in healthy adults in Pakistan, focusing on immune responses nine months post-vaccination.
  • A total of 173 participants aged 18-25 received either Sinopharm BBIBP-CorV or Pfizer-BioNTech vaccines, with antibody levels measured to compare immune responses.
  • Results show that the Pfizer group had significantly higher IgG antibody levels compared to the Sinopharm group, suggesting that a third Pfizer dose after two Sinopharm doses enhances overall immune response, with no significant differences between genders.

In this work, poly(styrene)@poly(-isopropylmethacrylamide--2-(,-dimethyl)aminoethyl methacrylate [p(sty)@p(NIPMAM-DMAEMA)] core/shell microgel particles were produced by a two-step free-radical precipitation polymerization process. Ag nanoparticles were successfully embedded inside the sieves of a crosslinked network by using silver nitrate as the precursor salt and NaBH as the reductant. The synthesized pure and hybrid microgels were analyzed by various characterization tools, including Fourier transform infrared (FTIR) and UV-visible (UV-vis) spectroscopies, transmission electron microscopy (TEM) and dynamic light scattering (DLS).

This study aimed to understand the lives of adolescents living in squatter settlements of Karachi, Pakistan regarding their assigned roles and responsibilities, health and social issues, their decision-making process, and mechanism and channels of information. An exploratory descriptive qualitative study design was employed. Purposive sampling techniques were used to carry out Focus Group Discussions (FGDs) with adolescents (n = 10, Participants = 190), adolescents' parents (n = 10, Participants = 180), and In-Depth Interviews of with key adolescent stakeholders (n = 20).

Article Synopsis
  • Dual immune checkpoint blockade (ICB) using CTLA4 and PD-(L)1 inhibitors shows improved anti-tumor effectiveness and immune toxicity compared to PD-(L)1 inhibitors alone in advanced non-small-cell lung cancer (NSCLC) patients.
  • Patients with mutations in STK11 and/or KEAP1 genes benefit more from the combination treatment compared to those receiving only PD-(L)1 inhibitors, as shown in the POSEIDON trial.
  • The loss of KEAP1 serves as a strong predictor for the success of dual ICB, as it leads to a more favorable outcome by changing the tumor's immune environment to better engage CD4 and CD8 T cells for anti-tumor activity. *

Neoantigen immunoediting drives immune checkpoint blockade efficacy, yet the molecular features of neoantigens and how neoantigen immunogenicity shapes treatment response remain poorly understood. To address these questions, 80 patients with non-small cell lung cancer were enrolled in the biomarker cohort of CheckMate 153 (CA209-153), which collected radiographic guided biopsy samples before treatment and during treatment with nivolumab. Early loss of mutations and neoantigens during therapy are both associated with clinical benefit.

Cytoplasmic aggregation of the TAR-DNA binding protein of 43 kDa (TDP-43) is the hallmark of sporadic amyotrophic lateral sclerosis (ALS). Most ALS patients with TDP-43 aggregates in neurons and glia do not have mutations in the TDP-43 gene but contain aberrantly post-translationally modified TDP-43. Here, we found that a single acetylation-mimetic mutation (K82Q) near the TDP-43 minor Nuclear Localization Signal (NLS) box, which mimics a post-translational modification identified in an ALS patient, can lead to TDP-43 mislocalization to the cytoplasm and irreversible aggregation.

Due to rapid urbanization, there have been continuous environmental threats from different pollutants, especially from microplastics. Plastic products rapidly proliferate significantly contributing to the occurrence of micro-plastics, which poses a significant environmental risk. These microplastics originated from diverse sources and are characterized by their persistent and widespread occurrence; human health and the entire ecosystem are adversely affected by them.

Background: Despite a large burden of life-limitingillness, there exists a dearth of services of palliative care in Pakistan. International guidelines have questionable applicability in Pakistan due to the socioeconomic differences. We generated a protocol describing the process of developing comprehensive palliative care guidelines and palliative care referral pathways for primary care practitioners to adopt in Pakistan.

Psychiatric disorders are highly prevalent in Pakistan and burdens the scarce number of psychiatrists present in the country. The establishment of evidence-based clinical practice guidelines (EBCPGs) and primary-care referral pathways within the local context is imperative to make the process efficient. In this Health Policy, we aimed to develop EBCPGs and primary-care referral pathways that are specific to Pakistan's primary-care setting, with the aim of facilitating the management of psychiatric conditions.

Introduction: Globally, the coronavirus disease 2019 (COVID-19) pandemic tested the resilience of the health system and its shock-absorbing capacity to continue offering healthcare services. The available evidences does not provide comprehensive insight into primary health care (PHC) system functioning across low- and middle- income countries (LMICs) during the pandemic. Therefore, the objective of this scoping review was to generate evidence on the resilience of PHC systems in LMICs during the COVID-19 pandemic.
