Spinal anesthesia (SA) for lumbar disk surgery in the genu-pectoral position (GP) has been proposed as an alternative to general anesthesia (GA). This study compares the haemodynamic effects of GP in two groups of patients undergoing either SA (n = 43) or GA (n = 40). Mean arterial pressure (MAP) and heart rate (HR) were recorded before and after GP.

Lumbar disc hernia is a frequent pathology whose treatment remains essentially surgical. The type of anesthesia can be general anesthesia with intubation and controlled ventilation, both mandatory due to the position, peridural or spinal anesthesia. The usual position is knee-chest position which impairs circulation and ventilation if some precaution are not taken.

This article reviews and summarizes the research conducted following publication of the 1980s American Association of Critical-Care Nurses' clinical research priorities. Original research conducted on the clinical priority topics between 1981 and 1991 was included. Review articles, doctoral dissertations, theses and abstracts were excluded unless judged to provide important information on the topic.

The American Association of Critical-Care Nurses completed a three-phased process of priority identification for critical care nursing research. In phase 1, a 78-item survey was generated following a comprehensive review of potential research topics. In phase 2, approximately 1000 critical care nurses rated each item on the survey for importance to critical care nursing.

Cardiac invalidism is a term used repeatedly in the literature during the past few decades, but little attention has been given to studying the variables contributing to this phenomenon. Data were collected from 111 patients with first acute myocardial infarction. Patients were visited at home 1 month and 4 months after hospital discharge.

Objective: To evaluate the clinical safety of heparin titration and the procedural cost of anticoagulation measurement using bedside low-range activated clotting time.

Design: Quasi-experimental study using data gathered through retrospective record review.

Setting: Coronary care, medical intensive care and telemetry units of a community hospital.

Objective: To determine if overprotection on the part of the patient's family and friends contributes to the development of cardiac invalidism after acute myocardial infarction.

Design: Longitudinal survey.

Setting: Nine hospitals in the southwestern United States.

Philosophies of science are perhaps the most covert yet significant forces influencing the direction of change within disciplines. Although the era of logical positivism has waned for many disciplines, newer philosophies may not be satisfactory, especially for the applied disciplines. This article describes an alternative philosophy of science with special significance for nursing.

Exposure to faculty is assumed to positively influence critical thinking ability and professional values; therefore, faculty must be assumed to be superior in these characteristics. As a first step toward testing these assumptions, this study used a cross-sectional survey technique to compare critical thinking ability and values in ADN-prepared (n = 32) and BSN-prepared (n = 32) registered nurses, nursing faculty (n = 32), and sophomore college students (n = 32) beginning a baccalaureate degree program in nursing. The critical thinking ability of faculty was not significantly higher than that of sophomore nursing students when the influence of age was controlled statistically.

The purpose of this study was to determine the effect of site of nitroglycerin ointment placement on headache and flushing in healthy subjects. Twenty-six subjects were given two doses of nitroglycerin ointment 3 hr apart in a randomly assigned arm-chest sequence or chest-arm sequence. Subject reports of headache and flushing using the visual analogue scale were recorded.

The advantage of calcium antagonist on cerebral arterial spasm during the peri-operative period has been recently demonstrated. However, the role of volemia and cardiac output in the pathogenesis of arterial spasm seems predominant. The basal hemodynamic profile of 17 patients operated for an intracranial aneurysm was investigated.

A moderate controlled hypotension has been performed with isoflurane in the presence of nimodipine (4 mg.h-1) on ten patients anesthetized for cerebral aneurysm clipping. The administration of both drugs induced a dose-dependent hypotension secondary to the reduction of the systemic vascular resistance without impairment of cardiac index.

No comprehensive theory of social support and psychological adjustment to chronic illness such as coronary heart disease (CHD) exists. This article reviews the literature on social support and adjustment to CHD from the perspective of Johnson's behavioral system model. It is argued that the quality of social support or nurturing is the major factor predicting cardiac crippled behaviors or dependency following myocardial infarction (MI).

Fifty-six patients with cardiac disease participated in a study to determine the effect of site placement on patient reports of headache and flushing after topical nitroglycerin ointment placement. Nitroglycerin was placed on the upper arm, chest, and pelvis in random order. Approximately 30 minutes after application, patients were asked to evaluate their headache and flushing using a visual analogue scale.
