Background: Arthroscopic rotator cuff repair (ARCR) is among the most commonly performed orthopaedic procedures. Several factors-including age, sex, and tear severity-have been identified as predictors for outcome after repair. The influence of the tear etiology on functional and structural outcome remains controversial.

Conservative treatment of Rockwood type III acromioclavicular joint separation: a randomized controlled trial sling vs. brace.

Background: Management of Rockwood III acromioclavicular joint separations is a matter of ongoing debate, with nonoperative treatment being favored in recent literature. The aim of this study is to compare clinical and radiological outcomes of nonoperative treatment with a brace, which elicits a direct reduction force to the distal clavicle, to a sling. We hypothesized the brace might yield in better acromioclavicular joint (ACJ) reduction and cosmesis.

Reverse Total Shoulder Arthroplasty in Patients Who Exceeded Their Life-Expectancy: A Retrospective Study.

Background: Reverse total shoulder arthroplasty (RTSA) has become an established treatment for cuff arthropathy, severe osteoarthritis and in certain fracture cases. Due to the increasingly aging population, patients who have already exceeded their life-expectancy pose a significant challenge to the shoulder surgeon.

Methods: Patients older than 83 years who received a RTSA were included.

Maxillary expansion or contraction and occlusal contact adjustment: effectiveness of current aligner treatment.

Objectives: To evaluate the precision of aligner (Invisalign®) treatment with the current material (SmartTrack®) in achieving expansion or contraction of the maxilla and occlusal contacts as simulated in the proprietary planning software (ClinCheck®, CC).

Materials And Methods: Thirty patients thus treated were retrospectively evaluated. Four maxillary models were analyzed per patient: a pretreatment model, a scan-based CC model, a posttreatment clinical model, and a CC model reflecting the treatment outcome as initially simulated.

Early locking plate removal following open reduction and internal fixation of proximal humeral fractures could prevent secondary implant-related complications.

Introduction: Proximal humeral fracture is a common cause of morbidity in the elderly and poses a challenge for the orthopedic surgeon. Open reduction and internal fixation (ORIF) with a locking plate is associated with high rate of secondary implant-related complications (IRC). Early implant removal could potentially reduce the risk of IRC and further improve the outcome in relatively asymptomatic patients.

The first 100 patients treated with a new anatomical pre-contoured locking plate for clavicular midshaft fractures.

Background: Pre-contoured locking plates were recently introduced in the management of clavicular midshaft fractures. These plates may offer advantages such as no necessity for intraoperative bending and reduced plate irritation. The purpose of this study was to review the clinical and radiographical outcome of the first 100 patients treated with a new anatomical pre-contoured locking plate.

Reverse total shoulder arthroplasty for acute head-splitting, 3- and 4-part fractures of the proximal humerus in the elderly.

Background: Anatomic reduction and stable internal fixation of complex proximal humeral fractures in the elderly is challenging. Secondary displacement, screw perforation, and humeral head necrosis are common complications. The outcome of hemiarthroplasty is unpredictable and strongly dependent on the uncertain healing of the greater tuberosity.

Isolated hypoplastic circumflex coronary artery: a rare cause of haemorrhagic myocardial infarction in a young athlete.

Hypoplastic coronary artery disease is a rare condition that may lead to myocardial infarction and sudden death. Here we describe for the first time an isolated hypoplasia of the left circumflex artery (LCX). An otherwise healthy and athletically active 16-year-old boy was admitted to the intensive care unit (ICU) after out-of-hospital cardiac arrest.

The effect of intramedullary bone endoscopy on the endosteal blood supply in long bones. An experimental study in sheep.

This study investigated whether the Intramedullary Bone Endoscopy (IBE) procedure within the cavity of an intact long bone will interfere with the local endosteal blood supply. In a sheep model, 10 animals underwent the IBE procedure with complete perioperative anaesthesiology monitoring. After the femora were harvested, histological analysis was performed to examine destruction of the endosteum and consecutive reduction in perfusion.

Acute subepicardial infarction associated with severe septic shock--insight in myocardial perfusion.

Isolated infarctions of the subepicardial myocardium without changes in subendocardium are extremely rare. We present an autoptic case with an acute subepicardial infarction of the right- and left-ventricular myocardium. A 53-year-old male was admitted to hospital with acute upper abdominal pain.

Fat marrow embolism during intramedullary bone endoscopy: an experimental study in sheep.

We present a study designed to investigate whether the intramedullary bone endoscopy (IBE) procedure within the cavity of an intact long bone will create embolic loads on the lungs similar to that of other orthopedic procedures (e.g., stem implantation in total hip arthroplasty [THA]).

The M.E Müller straight stem prosthesis: 15 year follow-up. Survivorship and clinical results.

The aim of the study was to document the long-term clinical and radiologic results as well as the 15 year survival of a single cemented straight stem. Our series consisted of consecutive patients with 161 straight stem prostheses implanted between 1984 and 1987 in 152 patients. Follow-up was performed prospectively at 1, 2, 5, 10 and 15 years post-operatively.

Conservative treatment of an acute compartment syndrome of the thigh.

Compartment syndromes of the thigh after blunt trauma without any fracture are rare. Most surgeons recommend operative treatment. There are different rules for compartment syndromes of the thigh in young athletes after blunt trauma compared to compartment syndromes at other locations [(1) the large volume of the quadriceps muscle, (2) its relatively elastic fascia, (3) the direct proximal contact to the hip muscles which allows extravasation of fluid out of the compartment)].

Frequent high-level expression of the immunotherapeutic target Ep-CAM in colon, stomach, prostate and lung cancers.

Epithelial cell adhesion molecule (Ep-CAM; CD326) is used as a target by many immunotherapeutic approaches, but little data are available about Ep-CAM expression in major human malignancies with respect to level, frequency, tumour stage, grade, histologic tumour type and impact on survival. We analysed by immunohistochemical staining tissue microarrays with 4046 primary human carcinoma samples from colon, stomach, prostate and lung cancers for both frequency and intensity of Ep-CAM expression under highly standardised conditions. A total of 3360 samples were analysable.

Generalized herpes simplex virus infection in an immunocompromised patient--report of a case and review of the literature.

Patients with immunodeficiency or treatment-related immunosuppression are at an increased risk of developing severe herpes simplex virus (HSV) infection. We present a fatal case of a generalized HSV-1 infection in a 22-year-old female afflicted by acute lymphoblastic leukemia who was treated with polychemotherapy. The terminal clinical course was characterized by abdominal pain, progressive hepatic failure, and disseminated intravascular coagulation.

Healing of the aponeurosis during recovery from aponeurotomy: morphological and histological adaptation and related changes in mechanical properties.

Aponeurotomy, which is the transection of an aponeurosis perpendicular to its length, is performed to lengthen spastic and/or short muscles. During recovery, the cut ends of the aponeurosis are reconnected by new connective tissue bridging both ends. The aim of this study is to investigate the histological features of this new connective tissue as well as its mechanical properties after recovery from aponeurotomy.

Five-year results of two cemented hip stem models each made of two different alloys.

Introduction: The aim of this study was to compare the influence of two different cemented hip stems each made of two different alloys concerning survival and outcome.

Materials And Methods: The 5-year results with 161 Mueller straight stems made of cobalt chromium (SS CoCr), 272 Mueller straight stems made of titanium (SS Ti), 233 Mueller SL stems made of titanium (SL Ti) and 255 SL stems made of cobalt chromium (SL CoCr) implanted consecutively in this order were compared. All patients were followed up prospectively during the first 5 years after implantation.

Granulicatella (Abiotrophia) adiacens infection associated with a total knee arthroplasty.

We describe the first case of prosthetic joint associated infection due to Granulicatella adiacens (formerly Abiotrophia adiacens). Diagnosis was made by broad spectrum PCR, and later by culture. Diagnosis and treatment of this microorganism is difficult.

Development of granulomatous common variable immunodeficiency subsequent to infection with Toxoplasma gondii.

Common variable immunodeficiency (CVID) is a heterogeneous immunodeficiency that is accompanied by granulomatous lesions in 5-10% of cases. Why some patients develop granulomatous disease remains unclear. Here we describe a 12-year-old previously healthy girl who presented with pancytopenia and granulomatous lymphoproliferation subsequent to infection with Toxoplasma gondii.

Dieulafoy lesion in a one-year-old child.

The Dieulafoy lesion is a rare cause of severe upper gastrointestinal bleeding associated with a significant mortality. Rebleeding from undiscovered lesions is frequent and often fatal. The outcome depends on a high degree of suspicion for the condition.

Ten-year follow-up of primary straight-stem prosthesis (MEM) made of titanium or cobalt chromium alloy.

Introduction: An increased revision rate of cemented titanium stems compared with cobalt chromium stems has been described after a short follow-up. In this paper, the minimum 10-year outcome after the implantation of titanium (Ti) and cobalt chromium ME (CoCr) Mueller straight-stem prostheses is analyzed in order to estimate the prognosis of the patients who were not revised.

Vascular diffusion density and survival of patients with primary lung carcinomas.

Aim: To investigate the clinical significance of tumour vascularisation in operated lung cancer patients.

Materials And Methods: Histological slides obtained from 498 patients with potentially curative operated lung carcinomas in two different institutions of thoracic surgery were immunohistochemically stained with an anti-CD34 antibody and subjected to quantitative image analysis. Syntactic structure analysis measured the absolute and relative features of vessels, including the numerical tumour cells densities relative to their nearest neighbouring vessel.

Experimental use of an albumin-glutaraldehyde tissue adhesive for sealing tracheal anastomoses.

Aim: The purpose of this study was to test the performance of an albumin-glutaraldehyde tissue adhesive (BioGlue, manufactured by CryoLife Inc., Kennesaw, GA, USA) when used on tracheal resections in rabbits, which is a sensitive model to investigate the biocompatibility of the glue.

Methods: The 24 animals were anesthetized and underwent cervicotomy with resection of a 10 mm long tracheal segment.
