: To study and evaluate the change in retinal nerve fiber layer (RNFL) and macular thickness and its clinical correlation with visual outcome following amblyopia therapy. : In this prospective interventional study, children with moderate to severe amblyopia were given optical correction and part-time occlusion therapy. Visual assessment and optical coherence tomography (OCT) were performed at days 0 and 3, 6 months of follow-up.

A young male in his late 20s presented with brownish discolouration of the conjunctiva and periocular area of both eyes. He was diagnosed as a case of lepromatous leprosy with recurrent type II lepra reaction 4 years ago and was started on multidrug therapy-multi bacillary, which included clofazimine. The best-corrected visual acuity was 20/20 in both eyes.

Background: The present study aimed to compare the efficacy, safety, and cost-effectiveness of quick penetrating solution (QPS) heparin, QPS diclofenac, and heparin gel in the prevention of superficial thrombophlebitis (ST).

Materials And Methods: This randomized controlled trial was conducted after approval from the Institutional Ethics Committee and registration to Clinical Trial Registry of India. Patients of 18-60 years age, I/II, and who needed venous cannulation for at least 72 h were included in the study.

Article Synopsis
  • This study looked at ways to prevent blood clots during and after major surgeries, which is important because clots can be dangerous.
  • Researchers compared different medications called factor Xa inhibitors and low molecular weight heparins (LMWHs) to see which ones worked best.
  • They found that rivaroxaban, fondaparinux, edoxaban, and bemiparin were better than enoxaparin in preventing blood clots, but fondaparinux and bemiparin increased the risk of serious bleeding more than the others.

  • More women of childbearing age are surviving heart transplants and many wish to become pregnant, but there's little information on their counseling experiences or knowledge regarding pregnancy after transplantation.
  • A survey of 64 women who underwent heart transplants revealed that many felt they lacked adequate information about post-transplant pregnancy and contraception before and after the procedure.
  • The results indicate a significant gap in communication between healthcare providers and these women, suggesting a need for improved education and support regarding reproductive health after heart transplantation.*

Article Synopsis
  • A study at Duke University Hospital reviewed 421 patients with left ventricular assist devices (LVADs) and implantable cardioverter-defibrillators (ICDs) to assess the frequency and causes of ICD shocks post-surgery.
  • Out of these patients, 33.9% experienced at least one shock, with 77.3% deemed appropriate and 22.7% inappropriate, primarily due to supraventricular tachycardia.
  • The findings suggest that better ICD programming, such as longer detection delays and higher rate cutoffs, could reduce the number of inappropriate shocks in LVAD recipients.

Life expectancy of patients with a durable, continuous-flow left ventricular assist device (CF-LVAD) continues to increase. Despite significant improvements in the delivery of care for patients with these devices, hemocompatability-related adverse events (HRAEs) are still a concern and contribute to significant morbility and mortality when they occur. As such, dissemination of current best evidence and practices is of critical importance.

Study post treatment improvement of Laryngopharyngeal Reflux Disease (LPRD) using non-invasive tools of Reflux symptom index (RSI), Reflux finding score (RFS) grading of videolaryngostroboscopy (VLS) and voice analysis. This study from December 2020 to April 2022 enrolled 100 adults with complaints suggestive of reflux symptoms and having Reflux Symptom Index (RSI) more than 13. All patients underwent VLS along with voice analysis.

Article Synopsis
  • Heart transplantation (HT) has traditionally used cold storage for preserving donor hearts, which poses risks like ischemic damage and poor outcomes after the transplant.
  • New preservation methods, such as the SherpaPak Cardiac Transport System and Transmedics Organ Care System, are being implemented and allow for better heart preservation, expanding donor options and geographic reach for procurement.
  • The paper discusses emerging evidence linking these new techniques to better outcomes post-transplant and recommends best practices for choosing between different preservation methods.

Importance: Left ventricular assist devices (LVADs) enhance quality and duration of life in advanced heart failure. The burden of nonsurgical bleeding events is a leading morbidity. Aspirin as an antiplatelet agent is mandated along with vitamin K antagonists (VKAs) with continuous-flow LVADs without conclusive evidence of efficacy and safety.

Background: Hepatitis C virus (HCV) nucleic acid amplification test (NAAT)-positive donors have increased the organ pool. Direct-acting antivirals (DAAs) have led to high rates of treatment success and sustained virologic response (SVR) in recipients with donor-derived HCV infection without significant adverse effects, although variability remains in the timing and duration of antivirals.

Methods: This retrospective study analyzed all adult HCV-NAAT-negative transplant recipients who received an organ from HCV-NAAT-positive donors from November 24, 2018, to March 31, 2022, at Duke University Medical Center with protocolized delay of DAA initiation until after hospital discharge, with at least 180-d follow-up on all patients.

Article Synopsis
  • Improved survival rates after heart transplantation (HT) have led more recipients to consider pregnancy, but there is limited data on the associated outcomes.
  • In a study analyzing over 19 million deliveries in the U.S., HT recipients were found to have significantly higher risks for severe maternal morbidity (SMM), preterm birth, and readmission within one year after delivery compared to non-HT recipients.
  • The findings indicate that women with a history of HT face 16 to 38 times greater odds of suffering from various complications during and after pregnancy, highlighting the need for careful counseling for these patients.

Background: Viral hemorrhagic fevers are becoming increasingly common in the tropics and subtropics. Dengue fever is currently the most important arthropod-borne viral disease because of its widespread distribution in more than 100 countries and its potential for extensive outbreaks of life-threatening disease.

Material And Methods: This study was a hospital-based cross-sectional study conducted in the Microbiology Laboratory of Maternal and Child Tertiary Care Hospital in Western Rajasthan, India, between January 2021 and December 2021.

Metastatic Renal Cell Carcinoma rarely presents in head and neck and is even rarer in the sinonasal region. However, a sinonasal metastatic mass is usually of RCC origin. These metastases may present prior to the renal symptoms or may appear after primary treatment.

Article Synopsis
  • Heart transplantation has been expanded to include donation after circulatory death (DCD) donors using the Organ Care System (OCS), addressing the shortage of available organs.
  • A study analyzed the effects of different procurement and transportation methods on heart transplant outcomes by comparing patients receiving DCD-OCS, brain dead donation (DBD-OCS), and DBD-cold storage organs through cardiac MRI imaging.
  • Although DCD transplants showed higher rates of primary graft dysfunction, there were no significant differences in mortality, rejection rates, or early cardiac MRI findings among the transplant groups.
