Introduction: Breast cancer is one of the most common types of cancer affecting women. Despite advancements in early diagnosis, neoadjuvant therapy, and various treatment modalities, mastectomy remains a common procedure for many women. Although some women opt for reconstructive surgery (BR), many do not have the indication, desire, or opportunity to undergo this procedure.

Breast surgery has evolved from mastectomy to breast-conserving surgery (BCS). Breast oncoplastic surgery later emerged with the inclusion and development of techniques used in plastic surgery for breast neoplasms. Recently, a new paradigm has been considered for mastectomy candidates with large multifocal and multicentric tumours, designated extreme oncoplasty (EO), which has allowed new techniques to be applied to tumours that would have been ineligible for BCS before.

Article Synopsis
  • Oncoplastic surgery (OS) aims to enhance the aesthetic results of breast-conserving surgery (BCS), but there is limited research on what causes unsatisfactory cosmetic outcomes in patients.
  • A study evaluated 300 patients who underwent breast surgery, analyzing both self-evaluations and photographic assessments of cosmetic results, focusing on various factors such as age, BMI, tumor size, and surgical specimen weight.
  • Results showed that younger patients, those with larger tumors, and specific patient characteristics were more likely to report unsatisfactory cosmetic outcomes, highlighting the need for better considerations in surgical planning.

Thoracoabdominal (TA) flaps are a good option for primary closure of small and medium defects after mastectomy for locally advanced breast tumours. Although they have a higher rate of necrosis than myocutaneous flaps, they can be easily performed by breast surgeons. Few studies on this procedure have been reported, and we have been unable to identify any prior publications reporting breast reconstruction with TA flaps.

Article Synopsis
  • Paget's disease of the breast (PDB) is a rare condition that requires the removal of the entire nipple-areolar complex, with surgical options traditionally including mastectomy or quadrantectomy.
  • A study in a Brazilian oncological hospital assessed the effectiveness of oncoplastic breast surgery (OBS) on patients with PDB, analyzing factors related to its use and its impact on local recurrence and survival.
  • The results indicated that while nearly 70% of patients underwent OBS, this did not significantly affect local recurrence or survival rates, suggesting that OBS offers improved surgical options without compromising patient outcomes.

Lymph node microcalcifications are rare events, and when they are accompanied by neoplasia, they usually seem to be associated with a metastatic condition. We present a case of a patient with breast cancer and lymph node microcalcifications undergoing neoadjuvant chemotherapy (NCT). A change in the calcification pattern towards becoming coarse was observed.

Breast Oncoplastic Surgery (OS) has established itself as a safe procedure associated with the treatment of breast cancer, but the term is broad, encompassing procedures associated with breast-conserving surgeries (BCS), conservative mastectomies and fat grafting. Surgeons believe that OS is associated with an increase in quality of life (QOL), but the diversity of QOL questionnaires and therapeutic modalities makes it difficult to assess from the patient's perspective. To answer this question, we performed a search for systematic reviews on QOL associated with different COM procedures, and in their absence, we selected case-control studies, discussing the main results.

Locally advanced ulcerated breast neoplasm is a condition that frequently occurs in developing countries. Generally, it is centrally localized and submitted to neoadjuvant therapy; thereafter, upon disappearance of the ulceration, it is submitted to radical mastectomy. Presence of axillary infiltration and ulceration with incomplete response makes it necessary for the use of surgical flaps for skin closure.
