Article Synopsis
  • * It found that 269 patients (3.1%) had C. acnes endocarditis, primarily affecting younger individuals with native valves, while prosthetic valve cases were more frequent in older age groups.
  • * Although survival rates were better for native valve cases, the incidence of C. acnes endocarditis is concerning and requires specialized diagnostic methods, but surgical outcomes were generally acceptable.

Aortic valve replacement improves and prolongs lives of patients with aortic valve disease, but requires significant healthcare resources, which are mainly determined by the length of associated hospital stays. Therefore, this study aims to identify risk factors for extended length of stay after surgical aortic valve replacement. Between 2018 and 2023, 458 consecutive patients underwent isolated surgical aortic valve replacement at our center and were included in our analysis.

Prosthesis-patient mismatch (PPM) is a common phenomenon after transcatheter aortic valve implantation (TAVI), especially in patients with small aortic annuli. Whether factors during implantation, such as the implantation depth, have an impact on the occurrence of PPM is currently unclear. The objectives of our study were to (1) investigate the influence of procedure planning- and implantation-related factors on the occurrence of PPM and (2) evaluate the impact of PPM on long-term mortality after TAVI.

Article Synopsis
  • A study in Germany compared two treatments for heart failure patients with secondary mitral regurgitation: transcatheter edge-to-edge repair and surgical mitral-valve repair or replacement.
  • The trial involved 210 patients and assessed outcomes like death, hospitalizations, and major adverse events, finding that transcatheter repair had similar efficacy but significantly fewer safety issues than surgery.
  • Results showed that transcatheter edge-to-edge repair was noninferior to the surgical approach, indicating it might be a safer alternative for this patient group.

Objective: Closure of the left atrial appendage (LAA) is a routine part of atrial fibrillation ablation surgery and significantly reduces stroke rates. Different LAA-closure techniques are used in cardiac surgery with variable results reported. We therefore evaluated the efficacy of 4 different LAA-closure techniques in patients undergoing cardiac surgery.

  • Transcatheter mitral valve replacement (TMVR) is a new treatment for mitral regurgitation that was studied for its effects on heart function using noninvasive pressure-volume loops.
  • A study of 26 patients showed improvements in left ventricular (LV) function after TMVR, indicated by a decrease in pressure-volume measures at discharge, but right ventricular (RV) function remained largely unchanged.
  • Long-term outcomes suggested that improvements in LV function post-TMVR were linked to lower 1-year rates of all-cause mortality and heart failure hospitalizations.

Objectives: Engineered Heart Tissue (EHT) is a promising tool to repair heart muscle defects and can additionally be used for drug testing. Due to the absence of an in vitro vascularization, EHT geometry crucially impacts nutrient and oxygen supply by diffusion capacity. We analyzed cardiomyocyte survival in different EHT geometries.

Purpose: To assess the association between fluctuations of arterial carbon dioxide early after start of extracorporeal membrane oxygenation (ECMO) with intracranial hemorrhage (ICH) or ischemic stroke (IS).

Materials And Methods: This single-center retrospective study included patients who required ECMO for circulatory or respiratory failure between January 2011 and April 2021 and for whom a cerebral computed tomography (cCT) scan was available. Multivariable logistic regression models were fitted to evaluate the association between the relative change of arterial carbon dioxide (RelΔPaCO) and ICH, IS or a composite of ICH, IS, and mortality.

Objectives: Patients with atrial fibrillation (AF) have lower left atrial (LA) strain, which is a predictor for LA function. Here, we evaluated the prognostic value of LA strain to predict the rhythm outcome in patients with persistent AF undergoing LA cryoablation concomitant to minimally invasive mitral valve repair.

Methods: Between 01/2016 and 12/2020, 72 patients with persistent AF underwent LA cryoablation during minimally invasive mitral valve surgery.

Article Synopsis
  • Transfemoral transcatheter aortic valve implantation (TAVI) is now the primary treatment for severe aortic stenosis, and intravascular lithotripsy (IVL-TAVI) can improve access for patients with severe iliofemoral calcifications.
  • A study comparing IVL-TAVI (30 patients) to traditional transaxillary access (TAX-TAVI, 44 patients) showed that IVL-TAVI had higher technical success (93.3% vs 81.8%) and lower safety complications (10.0% vs 31.8%).
  • The findings suggest that IVL-TAVI is a safe and effective method for patients previously deemed ineligible for transfemoral access

Background: Among low-risk patients with severe, symptomatic aortic stenosis who are eligible for both transcatheter aortic-valve implantation (TAVI) and surgical aortic-valve replacement (SAVR), data are lacking on the appropriate treatment strategy in routine clinical practice.

Methods: In this randomized noninferiority trial conducted at 38 sites in Germany, we assigned patients with severe aortic stenosis who were at low or intermediate surgical risk to undergo either TAVI or SAVR. Percutaneous- and surgical-valve prostheses were selected according to operator discretion.

Background: Deep sternal wound infection (DSWI) remains a serious complication after coronary artery bypass grafting (CABG). We herein aimed to stratify diabetic patients who underwent CABG using bilateral internal mammary artery (BIMA) for levels of glycated hemoglobin A1C (HbA1c) and compare postoperative outcomes.

Methods: Between January 2010 and August 2020, 4,186 consecutive patients underwent isolated CABG at our center.

Article Synopsis
  • A study looked at how thickening of the wall between the heart's ventricles (called interventricular septal hypertrophy) might affect patients after a heart procedure called TAVI.
  • Researchers compared 1,033 patients who had TAVI, separating them into two groups: one with normal wall thickness and another with thicker walls.
  • They found that while thicker walls resulted in higher pressure after the procedure, both groups had similar success rates and complications, meaning that wall thickness didn't really change the overall outcome.

Prosthetic materials are a source of bacterial infections, with significant morbidity and mortality. Utilizing the bionic "Lotus effect," we generated superhydrophobic vascular prostheses by nanocoating and investigated their resistance to bacterial colonization. Nanoparticles were generated from silicon dioxide (SiO), and coated vascular prostheses developed a nanoscale roughness with superhydrophobic characteristics.

Background:  A significant impact of surgeons' experience on outcomes of off-pump coronary artery bypass (OPCAB) has been recognized through previous large-scale studies. However, a safe, effective, and concrete OPCAB training was yet to be identified. We evaluate a safety of our OPCAB training model with single left internal mammary artery (LIMA)-left anterior descending artery (LAD) as a reasonable first step.

Article Synopsis
  • Infections in heart devices need to remove all device parts, but it's tough for patients who need a pacemaker.
  • A study looked at 190 patients with infections who got treatment in three different ways: just removing the device, adding a new part during removal, or putting in a new device on the opposite side.
  • All methods worked well, but patients with just device removal had the highest chance of dying within a year compared to the other two methods, although no one died during the procedures.

Article Synopsis
  • - The study aimed to compare clinical characteristics and 30-day mortality rates between patients with de novo heart failure-related cardiogenic shock (HF-CS) and those with acute-on-chronic HF-CS, finding significant differences in outcomes.
  • - An analysis of 1,030 patients revealed that while traditional markers of severity were similar, acute-on-chronic HF-CS was linked to higher mortality rates (55.9% vs. 45.5%) and greater severity of shock.
  • - The results suggest that the chronicity of heart failure significantly influences the severity of cardiogenic shock, underlining the need to consider this factor in future clinical trials.
