We present a case of a professional basketball player who suffered from nonunion bilateral chronic tibial fractures, successfully treated with ultrasound-guided intraosseous injection with bone marrow aspirate concentrate. The patient was able to return to play full time after recovery from the procedure.

Background: Bennett's fracture, a fracture-dislocation of the base of the first metacarpal, poses significant challenges due to the unique biomechanics of the thumb's carpometacarpal (CMC) joint. Effective management is critical to restoring thumb function and preventing long-term complications such as arthritis and instability.

Objective: This article provides a comprehensive overview of Bennett's fracture, including its mechanism of injury, diagnostic considerations, and management strategies, with a focus on conservative and surgical options.

Background: Transcranial magnetic stimulation (TMS) interventions could feasibly treat stroke-related motor impairments, but their effects are highly variable. Brain state-dependent TMS approaches are a promising solution to this problem, but inter-individual variation in lesion location and oscillatory dynamics can make translating them to the poststroke brain challenging. Personalized brain state-dependent approaches specifically designed to address these challenges are needed.

Background: We evaluated the feasibility, safety, and efficacy of a 2D-planar robot for minimally supervised home-based upper-limb therapy for post-stroke hemiparesis.

Methods: The H-Man, end effector robot, combined with web-based software application for remote tele-monitoring were evaluated at homes of participants. Inclusion criteria were: strokes > 28 days, Fugl-Meyer Motor Assessment (FMA) > 10-60/66, presence of a carer and absence of medical contraindications.

Recent genetic studies have revealed that hemimegalencephaly (HME) is a multi-system disorder associated with germline or mosaic variants within the PI3K-mTOR-GATOR1 signaling pathways. Patients with HME typically develop drug-resistant epilepsy necessitating extensive evaluation, hemispherectomy, and long-term management. We describe the role of a multidisciplinary team (MDT) for the diagnosis and management of recent patients with HME at UCLA who underwent hemispherectomy.

Telephysiotherapy has emerged as a vital solution for delivering remote healthcare, particularly in response to global challenges such as the COVID-19 pandemic. This study seeks to enhance telephysiotherapy by developing a system capable of accurately classifying physiotherapeutic exercises using PoseNet, a state-of-the-art pose estimation model. A dataset was collected from 49 participants (35 males, 14 females) performing seven distinct exercises, with twelve anatomical landmarks then extracted using the Google MediaPipe library.

The Internet of Health Things (IoHT) is a broader version of the Internet of Things. The main goal is to intervene autonomously from geographically diverse regions and provide low-cost preventative or active healthcare treatments. Smart wearable IMUs for human motion analysis have proven to provide valuable insights into a person's psychological state, activities of daily living, identification/re-identification through gait signatures, etc.

Background/objective: The purpose of this study was to investigate the impact of the COVID-19 pandemic on mortality rates in a community-dwelling spinal cord injury (SCI) population in Ontario.

Methods: Using health administrative databases, monthly mortality rates were evaluated pre-pandemic, during the pandemic, and post-pandemic from March 2014 to May 2024. Data were stratified by sex, injury etiology, and mental health status.

Article Synopsis
  • There isn't much guidance about how to prescribe opioids for people with broken bones, especially in surgery, so this study wants to help doctors understand what patients need after surgery for tibial (shin bone) fractures.
  • The researchers looked at 1,814 patients who had tibial fractures from 2015 to 2022 and measured how much opioids (pain medicine) they used and for how long after their surgeries.
  • They found that patients with certain types of fractures, like tibial plateau fractures, needed more pain medicine, and those with health issues like kidney disease and high blood pressure also required more opioids to manage their pain.

Purpose: Injectable biologics have not only been described and developed to treat dermal wounds, cardiovascular disease, and cancer, but have also been reported to treat chronic pain conditions. Despite emerging evidence supporting regenerative medicine therapy for pain, many aspects remain controversial.

Methods: The American Society of Pain and Neuroscience (ASPN) identified the educational need for an evidence-based guideline on regenerative medicine therapy for chronic pain.

Physiotherapy plays a crucial role in the rehabilitation of damaged or defective organs due to injuries or illnesses, often requiring long-term supervision by a physiotherapist in clinical settings or at home. AI-based support systems have been developed to enhance the precision and effectiveness of physiotherapy, particularly during the COVID-19 pandemic. These systems, which include game-based or tele-rehabilitation monitoring using camera-based optical systems like Vicon and Microsoft Kinect, face challenges such as privacy concerns, occlusion, and sensitivity to environmental light.

Persons with disabilities experience numerous barriers to healthcare access including vaccine accessibility. The purpose of this study was to determine COVID-19 vaccine uptake in the spinal cord injury and disease (SCI/D) population of Ontario and identify potential factors influencing C OVID-19 vaccine uptake. This was a retrospective closed-cohort study using administrative health data on individuals with SCI/D of traumatic and non-traumatic causes to examine the monthly number of COVID-19 vaccine doses received between December 2020 and December 2023.

In recent years, innovative technologies have gained prominence in stroke patient rehabilitation worldwide, with virtual reality-based mirror therapy (VRBMT) emerging as a notable example. Hence, the present study aims to determine the efficacy of VRBMT on upper extremity motor function, manual performance, and gross manual dexterity among stroke patients. The systematic search utilized Population, Intervention, Comparison, Outcome, and Design (PICOD) paradigm, and the study searched was based on 2012-2023, utilizing different databases.

We study the process of value co-creation in physiotherapist-patient relationships and the underlying mechanisms of this process. We present the Googling Effect Model outlining the relationship between patients' pre-encounter online information search and their commitment to compliance in physiotherapy service exchange. We tested the model on a sample of physiotherapy service patients (n = 446) in the United States.

Colonoscopy is a reliable diagnostic method to detect colorectal polyps early on and prevent colorectal cancer. The current examination techniques face a significant challenge of high missed rates, resulting in numerous undetected polyps and irregularities. Automated and real-time segmentation methods can help endoscopists to segment the shape and location of polyps from colonoscopy images in order to facilitate clinician's timely diagnosis and interventions.

Integrated care systems join up health and care services, so that people have the support they need, in the right place, at the right time. The aims include improving outcomes in healthcare, tackling inequalities in access and enhancing productivity and value for money. This is needed for neuroscience care as the traditional delivery of neuroscience care is inefficient, outdated and expensive, and can involve complex referral pathways and long waiting times.

Background: Severe posterior glenoid bone loss with glenohumeral osteoarthritis with an intact rotator cuff can be managed with reverse shoulder arthroplasty but requires lateralization and version correction to avoid potential complications, such as instability, notching and implant failure. Angled bone grafting with humeral head autograft can provide durable glenoid bone stock, but results have been mixed. The purpose of this study was to evaluate patient-reported and objective outcomes as well as complication and failure rates for patients who underwent angled humeral head autografting for severe retroversion.
