Hazelnuts are frequently involved in IgE-mediated reactions and are the main cause of nut allergies in Europe. Most food products are processed before human consumption. Food processing can modify the structure, properties, and function of proteins, and as a result, the IgE-binding capacity of allergens can be affected.

Background: Anaphylaxis is a systemic allergic reaction that is potentially life-threatening. Occupational anaphylaxis is an anaphylaxis that occurs in an occupational context. In this position paper, we propose diagnostic criteria for occupational anaphylaxis and provide an overview of the current state of knowledge in terms of prevalence, triggers, prevention, and management.

Article Synopsis
  • Targeted therapies using biologics for atopic diseases, urticaria, and angioedema are advancing quickly, with several new antibodies developed, tested, and approved for clinical use, like omalizumab and dupilumab.
  • There is ongoing research into combining different biologics for enhanced treatment efficacy, expanding their applications to conditions like food allergies and eosinophilic esophagitis.
  • There are emerging concerns about unexpected side effects and hypersensitivity reactions associated with these therapies, raising important questions about their safety and mechanisms, particularly in specific patient groups like children.

Chronic spontaneous urticaria (CSU) is a debilitating, inflammatory skin condition characterized by infiltrating immune cells. Available treatments are limited to improving the signs and symptoms. There is an unmet need to develop therapies that target disease-driving pathways upstream of mast cell activation to inhibit or delay the progression of CSU and associated comorbidities.

Background: Eosinophilic esophagitis (EoE) is an inflammatory disease of the esophagus that belongs to the spectrum of Th2-mediated diseases. It is often associated with atopic comorbidities such as allergic asthma (AA) and poses a therapeutic challenge.

Case Report: We report on a 43-year-old patient with EoE and AA who did not show sufficient therapeutic control despite standard therapy.

Background: Anaphylaxis represents the most severe end of the spectrum of allergic reactions. Frequent elicitors of anaphylaxis are insects, foods, and drugs. This paper summarizes recent development with regard to emerging and novel elicitors of anaphylaxis.

The aim of this review is to present relevant trigger as well as augmentation factors that can induce or exacerbate urticaria on the basis of a current, PubMed-based literature search. In addition to a brief description of relevant influencing factors in acute and chronic inducible urticaria, the focus will be on chronic spontaneous urticaria. In particular, the aggravating role of medication, stress, food, psychological and metabolic comorbidities, infections and inflammation as well as hormonal processes will be discussed.

Article Synopsis
  • Concerns about allergic reactions to dental materials are increasing among patients, prompting a need to analyze current sensitization patterns.
  • A study of 2730 patients revealed that women, particularly those over 40, showed the highest sensitization rates to metals like nickel and palladium, as well as to (meth)acrylates.
  • The findings suggest that patch testing for contact allergies should also include (meth)acrylates and natural substances like propolis, rather than focusing solely on metals.

Background: Epidemiologic data on occupational anaphylaxis is scarce, and there is a need of more knowledge about work-related anaphylactic episodes.

Methods: Based on the data of the Anaphylaxis Registry, we identified cases related to occupational exposure and analyzed the elicitors, demographics, severity of clinical reaction and management.

Results: Since 2017, 5851 cases with an information about the occupational relation of the anaphylactic episode were registered whereby 225 (3.

Article Synopsis
  • Hymenoptera venom (HV), injected by bees and wasps during stings, can cause allergic reactions in about 3% of people, leading to severe local or systemic allergic reactions in some cases.
  • The guidelines suggest that while symptomatic therapy is needed after a severe local reaction, specific allergen immunotherapy isn't necessary unless severe reactions occur beyond the skin.
  • Clinicians should assess patients for risk factors, measure tryptase levels if reactions are significant, and test for specific IgE antibodies to diagnose sensitization to Hymenoptera venom.

Background: Anaphylaxis is a serious systemic reaction-data on fatal and near-fatal reactions are limited.

Objective: To better understand clinical patterns and risks factors of severe anaphylaxis by a deep analysis of data from fatal and near-fatal anaphylaxis.

Methods: Data from the European Anaphylaxis Registry on fatal/near-fatal anaphylactic reactions and national data on anaphylaxis fatalities were investigated.

Article Synopsis
  • - The integrated care pathways for atopic dermatitis (AD-ICPs) serve to connect existing treatment guidelines and expert insights into a structured plan that caters to different levels of AD severity and healthcare resources across various countries.
  • - Developed by the GA LEN ADCARE network and other stakeholders in 2020-2021, the AD-ICPs detail diagnostics, treatment options, and emphasize the roles of pharmacologists and other contributors in managing AD, particularly in pediatric cases.
  • - The initiative aims to enhance AD management through a multidisciplinary approach that addresses urgent needs like better access to care, specialist training, educational programs, and personalized treatments, ultimately leading to improved patient outcomes.

Background: Allergic medical care in Germany is organized on an interdisciplinary basis. An overview of the current care situation is necessary to manage and improve interdisciplinary cooperation.

Methods: Between January and February 2022, questionnaires were sent online and by mail to chief physicians of inpatient clinical departments to which most allergological diseases are assigned (dermatology, otorhinolaryngology [ENT], pulmonology, pediatrics, environmental/occupational medicine, gastroenterology; n = 899).

Background: Hair cosmetic products contain several, partly potent contact allergens, including excipients like preservatives. Hand dermatitis in hairdressers is common, scalp and face dermatitis in clients or self-users (summarised here as 'consumers') may be severe.

Objective: To compare frequencies of sensitization to hair cosmetic ingredients and other selected allergens between female patch tested patients working as hairdressers and consumers without professional background, respectively, who were tested for suspected allergic contact dermatitis to such products.

Vegan diets are currently attracting a great deal of attention. However, avoiding animal-based foods restricts the diet and is associated with risks, the extent and medical implications of which are at present not sufficiently understood. Elimination diets represent the usual therapeutic long-term management in the presence of food allergy.
