Article Synopsis
  • The EndoFLIP is a new diagnostic system that evaluates esophageal function in real time, especially beneficial for patients with unclear esophageal motor issues like outflow obstruction and achalasia.
  • The study at San Ignacio University Hospital involved 27 patients, primarily women, all over 18, and focused on cases with inconclusive prior diagnoses.
  • Results showed varied contractile responses in patients, revealing that many had normal or absent responses, which helps in understanding their individual esophageal conditions.

Background/aims: Peroral endoscopic myotomy (POEM) has been increasingly used for achalasia in Latin America, where Chagas disease is prevalent, and this makes POEM more challenging. The aim of this study was to determine the learning curve for POEM in Latin America.

Methods: Patients undergoing POEM in Latin America with a single operator were included from a prospective registry over 4 years.

Intraductal papillary neoplasm of the bile duct (IPNB) is a rare premalignant condition, defined as an epithelial neoplasm of the bile duct with exophytic papillary growth that can develop in any segment of the biliary tree. This pathology, with the highest prevalence in Asia, has been associated with the presence of hepatolithiasis and eastern liver infection (clonorchiasis). The diagnosis will depend on the clinical suspicion against the findings evidenced in the different diagnostic supports (invasive and non-invasive).

Article Synopsis
  • Both Heller myotomy (HM) and per-oral endoscopic myotomy (POEM) are effective treatments for achalasia, a condition that affects swallowing.
  • A study involving 133 patients from nine Latin American centers showed that POEM had a higher initial success rate than HM, and both treatments significantly reduced the Eckardt score, which measures symptoms.
  • In Chagas disease patients, POEM was notably more effective than HM, resulting in a greater drop in the Eckardt score and shorter hospital stays for those undergoing the procedure.
