Despite an increasing number of adults older than 60 years with psychiatric disorders, there are few studies on older patients in psychiatric emergencies and no European data. We aimed to describe the population of patients aged 60 years and older who presented to the main French psychiatric emergency centre and identify predictors of psychiatric hospitalization. This monocentric study included 300 consecutive patients aged 60 years and older.

Background: Migrants, and particularly asylum seekers, are at increased risk of psychiatric disorders in comparison with natives. At the same time, inequalities in access to mental health care are observed.

Methods: In order to evaluate whether the Parisian public psychiatric system is optimally structured to meet the needs of this population, we examined data on mental health and service use considering three different levels: the global system treatment level, a psychiatric reception center, and mobile teams specializing in access to psychiatric care for asylum seekers.

Since France was put into lockdown on 17 March 2020 in an unprecedented step to attempt to limit the spread of coronavirus, there has been a sharp fall in the number of psychiatric emergency admissions despite the COVID-19 pandemic which has increased stress levels. The first part of this study shows the reorganisation and adaptation of mental health services to meet the needs in the psychiatric department. The second part is a brief analysis of patient flows to CPOA and the presentation of different hypotheses explaining these disruptions during lockdown.

To date, we have no French data about the psychiatric consequences of the Covid-19 pandemic and the confinement. In the largest French psychiatric emergency centre, we compared the average number of consultations per day during the confinement with the same period preceding the confinement and with the same periods of previous years, and we observed a significant drop in attendance. Our team had to adapt promptly to these changes in public mental health services, and we set up a telephone hotline dedicated to psychiatric patients and their families, in order to prevent a secondary psychiatric crisis.

Emergency psychiatric units are receiving more and more foreign unaccompanied minors: for many of these young people it is their first contact with psychiatric services. This context of crisis crystallises the administrative and legal paradoxes at work in the support of these youngsters. Coordination between emergency care, statutory bodies and community-based care enables stability to be established while the care project is being set up.

The risk/benefit ratio weighs up on the one hand the expected advantages and on the other the potential risks of a procedure. It is taken into consideration with every therapeutic decision, whether it concerns medication-based treatments, psychotherapy or any other type of treatment. The ethical dimension is a critical element which must be taken into account during team deliberations, whenever these are possible.

This column reviews the evolution of French laws governing psychiatric commitment, culminating in the July 2011 Act, which was opposed by most professional organizations. The 2011 Act has maintained the two traditional French approaches to involuntary treatment: at the request of a third person and upon a decision by a prefect representing the government. However, the 2011 Act introduced major innovations into French practices: systematic review by a judge, a 72-hour observation period, and the possibility of compulsory community treatment.

Aims: Neuronal uncoupling proteins are involved in the regulation of reactive oxygen species production and intracellular calcium homeostasis, and thus, play a neuroprotective role. In order to explore the potential consequences of neuronal uncoupling proteins variants we examined their association in a sample of Caucasian patients suffering from schizophrenia and phenotyped them according to antipsychotic response.

Materials & Methods: Using a case-control design, we compared the frequencies of 15 genetic variants spanning UCP2, UCP4 and UCP5 in 106 French Caucasian patients suffering from schizophrenia and 127 healthy controls.

Investigations of memory impairment in schizophrenia have frequently revealed a strategic processing deficit at encoding. The authors studied an early encoding process, refreshing (in this case, thinking of a stimulus that has just-previously been presented), and its impact on recognition memory in schizophrenia. Following simultaneous presentation of three words or a single word in the top, middle, or bottom position of the screen, 25 patients with schizophrenia and 25 control participants saw and read a new word (read condition), or a word presented on the previous screen (repeat condition), or saw a dot indicating that they should think of and say the last word to have appeared in that position (refresh condition).

This retrospective chart review of a clinical cohort of 19 refractory schizophrenic or schizoaffective patients treated with maintenance electroconvulsive therapy addresses the indications for this treatment, its efficacy, and its impact on daily functioning and hospitalizations. Background: Polypharmacy without evidence-based support is sometimes needed for patients treated with 40% to 70% clozapine who are clozapine nonresponders. Several somatic augmentation strategies are proposed in the scientific literature, with different levels of evidence for safety and efficacy.

Objectives: The purpose of the present study is to review the available literature on the efficacy and safety of clozapine augmentation with somatic agents other than antipsychotics.

Background: Approximately 40% to 70% of neuroleptic-resistant schizophrenic patients are nonresponders to clozapine. Several clozapine augmentation strategies have come into clinical practice although often without evidence-based support. Among these strategies, the combined use of clozapine with another antipsychotic has been reported for up to 35% of patients receiving clozapine.

We assessed the subjective quality of life (QOL) of 30 deficit schizophrenic patients compared to 112 nondeficit schizophrenic patients. The deficit patients did not differ in term of QOL, total score of positive symptoms, general psychopathology from the nondeficit patients. This result suggested an absence of impact of primary negative symptoms on the subjective QOL in schizophrenic patients.
