Adrenal and pancreatic presentation of subdiaphragmatic retroperitoneal bronchogenic cysts.

Bronchogenic cysts are aberrations of normal development throughout the embryonic foregut; abdominal or retroperitoneal presentations are rare. They will often present a diagnostic dilemma because their appearance can mimic other, more common diagnoses. The initial work-up, differential diagnosis, management, and follow-up may present clinical challenges.

Laparoscopic adrenalectomy for large unilateral pheochromocytoma: experience in a large academic medical center.

Background: Laparoscopic adrenalectomy is the treatment of choice for most adrenal lesions. Concerns have persisted about its application to large pheochromocytomas due to reports of hemodynamic instability, difficult dissection, and tumor spillage.

Methods: Thirty patients underwent laparoscopic adrenalectomy for unilateral pheochromocytoma between 1998 and 2006.

Institute of medicine committee report on resident duty hours: a view from a trench.

Background: In late 2008, the Institute of Medicine (IOM) published a report recommending more restrictive limits on resident work hours to promote patient safety. Reaction from the graduate medical education community has focused on concerns about a lack of evidence supporting the IOM's recommendations. We highlight 3 concerns with the report: 1) a disproportionate attention to resident fatigue when changes in other areas may have a larger impact on patient safety.
