Indonesia has the third highest number of tuberculosis (TB) patients infected with Mycobacterium tuberculosis (MTB) Lineage 1 (L1). Most of these MTB L1 cases can be found in Indonesia's remote easternmost province of Papua, one of Indonesia's most underdeveloped provinces with a particularly high burden for TB. In this study, we sequenced and described 42 MTB L1 isolates from a well-characterized cohort of patients.

Background: Hydroxychloroquine (HCQ) has proved ineffective in treating patients hospitalised with Coronavirus Disease 2019 (COVID-19), but uncertainty remains over its safety and efficacy in chemoprevention. Previous chemoprevention randomised controlled trials (RCTs) did not individually show benefit of HCQ against COVID-19 and, although meta-analysis did suggest clinical benefit, guidelines recommend against its use.

Methods And Findings: Healthy adult participants from the healthcare setting, and later from the community, were enrolled in 26 centres in 11 countries to a double-blind, placebo-controlled, randomised trial of COVID-19 chemoprevention.

Article Synopsis
  • This study focuses on developing a core genome multilocus sequence typing (cgMLST) scheme for a pathogenic bacterium found in the human nasopharynx, addressing its genetic diversity.
  • To create and validate the cgMLST scheme, researchers analyzed a mix of complete reference genomes and high-quality draft genomes, resulting in a refined list of core genes essential for its metabolism and genetic functions.
  • The findings demonstrate that the core genome allelic profile effectively represents the genetic relationships among different isolates, thereby improving our understanding of this pathogen's population structure for future genomic analysis.

A strong and effective COVID-19 and future pandemic responses rely on global efforts to carry out surveillance of infections and emerging SARS-CoV-2 variants and to act accordingly in real time. Many countries in Southeast Asia lack capacity to determine the potential threat of new variants, or other emerging infections. Funded by Wellcome, the Southeast Asia initiative to combat SARS-CoV-2 variants (SEACOVARIANTS) consortium aims to develop and apply a multidisciplinary research platform in Southeast Asia (SEA) for rapid assessment of the biological significance of SARS-CoV-2 variants, thereby informing coordinated local, regional and global responses to the COVID-19 pandemic.

Recognizing the growing global burden of fungal infections, the World Health Organization established a process to develop a priority list of fungal pathogens (FPPL). In this systematic review, we aimed to evaluate the epidemiology and impact of invasive infections caused by Aspergillus fumigatus to inform the first FPPL. The pre-specified criteria of mortality, inpatient care, complications and sequelae, antifungal susceptibility, risk factors, preventability, annual incidence, global distribution, and emergence were used to search for relevant articles between 1 January 2016 and 10 June 2021.

Existing literature has portrayed numerous challenges that healthcare workers (HCWs) faced during the COVID-19 pandemic, such as heightened risks of transmission against the scarcity of protective equipment, burgeoning workload, and emotional distress, to name a few. However, most studies explored HCWs' experiences at the individual level rather than examining the collective responses. Exploring these experiences could reveal the social-cultural locality of the pandemic while identifying the system constraints in public health emergencies.

Background: The clinical management of leprosy is complicated by leprosy reactions (LR) causing irreversible nerve damage and disabilities. LR often require long-term use of corticosteroids causing serious side effects. Adjunct host-directed therapy (HDT) is a potentially attractive strategy in leprosy to prevent LR and associated immunopathology, modulate immunological memory that protects against recurrence, and thereby reduce nerve damage, disability and corticosteroid-associated morbidities.

Background: The WHO Global Antimicrobial Resistance Surveillance System (GLASS) aims to describe antimicrobial resistance (AMR) patterns and trends in common bacterial pathogens, but data remain limited in many low and middle-income countries including Indonesia.

Methods: We systematically searched Embase, PubMed and Global Health Database and three Indonesian databases for original peer-reviewed articles in English and Indonesian, published between January 1, 2000 and May 25, 2023, that reported antimicrobial susceptibility for the 12 GLASS target pathogens from human samples. Pooled AMR prevalence estimates were calculated for relevant pathogen-antimicrobial combinations accounting for the sampling weights of the studies (PROSPERO: CRD42019155379).

Article Synopsis
  • A study in southeast Asia analyzed the impact of SARS-CoV-2 variants and inactivated vaccines on COVID-19 severity and mortality from September 2020 to January 2023, covering pre-delta, delta, and omicron phases.
  • Findings revealed that while delta variant infections had a higher rate of severe cases (50.1%) and mortality (24.4%), the omicron variant showed significantly lower rates (15.2% for severe disease and 9.6% for mortality).
  • Vaccination significantly reduced the odds of severe disease (by 89% for delta and 98% for omicron), and vaccinated individuals experienced a lower likelihood of death compared to unvaccinated individuals (51% overall reduction

Article Synopsis
  • There's limited information on how diagnostic and antibiotic practices were used in Indonesia's healthcare during the COVID-19 pandemic, which is important for monitoring these practices in low and middle-income countries.
  • A study analyzed hospital data from 2019 to 2020, comparing COVID-19 patients to non-COVID-19 patients in terms of antibiotic use and blood culture practices.
  • Results showed more COVID-19 patients had blood cultures taken timely compared to non-COVID-19 patients, but overall, both groups had low rates of timely blood cultures, and COVID-19 patients had a longer duration of antibiotic treatment.

Case-based surveillance of antimicrobial resistance (AMR) provides more actionable data than isolate- or sample-based surveillance. We developed A Clinically Oriented antimicrobial Resistance surveillance Network (ACORN) as a lightweight but comprehensive platform, in which we combine clinical data collection with diagnostic stewardship, microbiological data collection and visualisation of the linked clinical-microbiology dataset. Data are compatible with WHO GLASS surveillance and can be stratified by syndrome and other metadata.
