The recording of mortality after acute myocardial infarction can be based on different data, some of which yield very different results. These differences are due to a number of factors, including the definition of acute myocardial infarction, patient selection and the methods used to determine mortality. While routine data are primarily used for billing purposes, procedural data for coronary angiography and percutaneous coronary intervention (PCI) are used for external quality assurance and therefore only include patients who undergo invasive diagnostic procedures.

The COVID-19 pandemic placed a significant burden on the German healthcare system. Based on the experience of severe disease progression of the SARS-CoV‑2 infection from neighboring European countries in the early 2020s, with ICU overload and high mortality rates, efforts were made in Germany to increase the capacity of available ICU beds. We report on a 67-year-old female patient with recurrent syncope induced by asystole and polymorphic ventricular tachycardia without relevant structural heart disease. After secondary prophylactic ICD implantation, beta-blocker and flecainide therapy, the patient suffered from recurrent syncope and ICD shocks due to ventricular fibrillation. A rare cause of polymorphic ventricular tachycardia in patients without structural heart disease was found and successfully treated.
