A 55 years old man was extubated on first postoperative day following coronary artery bypass grafting at 7:30 am. The same day at 5 pm, he became drowsy but arousable only on painful stimuli with severe generalized hypertonia and bilateral upgoing plantars. He was reventilated and a provisional diagnosis of cerebrovascular accident was made.

Article Synopsis
  • The study compares outcomes of double valve replacement (DVR) versus isolated mitral (MVR) or aortic valve replacement (AVR) for patients with rheumatic heart disease.
  • The study involved 493 patients followed over 12 years, analyzing survival rates, causes of mortality, and complications like valve thrombosis and bleeding.
  • Results showed overall good survival rates, with DVR having the highest survival at 87.4%, while isolated MVR and AVR had slightly lower rates, and complications varied across the groups.

Objectives: In this study, we compare the follow-up results of two types of tricuspid valve repair and review the results of no repair in moderately severe tricuspid regurgitation (TR) along with mitral valve replacement or mitral and aortic valve replacement.

Patients And Methods: This prospective follow-up study, carried out on 106 consecutive patients, was conducted at the Department of Cardiac Surgery, Punjab Institute of Cardiology, Lahore, from September 1994 through August 2008. The patients were divided into three groups: suture bicuspidization, 36 (34%); modified De Vega's repair, 47 (44.

Myxomas are rarely associated with congenital cardiac anomalies. We report a case of a young female presenting with symptoms of atrial myxoma but having co-existing ASD visualized on 64 slice multidetector computed tomography (MDCT) prior to cardiac surgery. Patient had a successful resection of biatrial myxoma and ASD repair.

Article Synopsis
  • Chronic total occlusion of the left main stem coronary artery is very rare, with only 17 cases identified out of over 67,000 angiograms conducted between 1998 and 2008.
  • Most patients were middle-aged men with risk factors like diabetes, hypertension, and smoking, undergoing coronary artery bypass grafting with a mix of arterial and venous grafts.
  • Post-surgery, there were some complications such as a myocardial infarction and transient ischemic attack, but no deaths occurred, indicating that surgical revascularization is a viable treatment option for this condition.

We report a 22-year-old mother of 2 who presented to us with exertional chest pain for 3 years. On coronary angiography she was diagnosed to be suffering from Bland White Garland syndrome (BWGS). She had a giant and grossly tortuous right coronary artery (RCA) forming collaterals with the left coronary artery (LCA), which was draining into the pulmonary artery (PA).

Purpose: To identify optimum international normalized ratio (INR) levels and required warfarin doses and anticoagulation-related complications in patients following mechanical prosthetic valve replacement.

Materials And Methods: Five hundred and seven patients were prospectively followed up for 10 years (2008.5 patient-years).

Article Synopsis
  • The study focused on patients with mechanical heart valves who underwent non-cardiac surgeries and how their anticoagulation therapy was managed.
  • It analyzed data from 507 patients over a 12-year period and found that 42 patients had various surgeries, mainly for abdominal issues and included emergency and planned operations.
  • The conclusions indicated that with proper management, these patients could safely undergo non-cardiac surgeries without significant complications related to their heart valves or anticoagulation.

Anticoagulation and proper INR (International Normalized Ratio) monitoring is essential for patients having mechanical heart valves; it is vital in these patients in order to prevent lethal complications such as valve thrombosis and systemic embolism. In pregnancy, it becomes even more important as pregnancy itself is a hypercoagulable state. This report describes a female patient having undergone mitral valve replacement with a Starr Edward metallic prosthesis.
