Minimally Invasive Chevron Surgery and Bosch Percutaneous Osteotomy in Hallux Valgus Treatment. Midterm Results. Comparative Radiological Study.

 The purpose of the present study is to compare the radiological results of angular correction and its maintenance in the medium term between two minimally invasive techniques for the treatment of hallux valgus (minimally invasive chevron surgery vs. Bosch technique).  A comparative prospective analysis of patients undergoing surgery for symptomatic hallux valgus deformity was performed.

[Complications in posterior ankle arthroscopy by classic and non-distraction portals].

Introduction: Posterior ankle arthroscopy allows the diagnosis and treatment of multiple pathologies through minimal trauma. The use of posterior portals associated or not to mechanical distraction, allows a good vision of the posterior ankle, tendons of the region, and subtalar joint.

Objectives: We evaluated intra- and postoperative complications of posterior ankle arthroscopy without joint distraction, and compared them with those described in international literature.

[Recurrent dislocation of peroneal tendons. Minimally invasive technique for the deepening of the groove].

Introduction: Since William Clancy (1979) and Richard Ferkel (1997) publications, the technique of peroneal groove deepening in patients with chronic dislocation, has been used with very satisfactory results and few complications. The aim of the present study is to describe a new minimally invasive technique of peroneal groove deepening with burs and specific percutaneous surgery instrumentation and without fibula osteotomy; and to evaluate the clinical and imaging results, in 5 patients operated with this technique.

Material And Methods: A retrospective study of 5 patients with clinical and imaging diagnosis of chronic dislocation of peroneal tendons.

Evidence-based clinical practice guidelines for the management of sedoanalgesia and delirium in critically ill adult patients.

Given the importance of the management of sedation, analgesia and delirium in Intensive Care Units, and in order to update the previously published guidelines, a new clinical practice guide is presented, addressing the most relevant management and intervention aspects based on the recent literature. A group of 24 intensivists from 9 countries of the Pan-American and Iberian Federation of Societies of Critical Medicine and Intensive Therapy met to develop the guidelines. Assessment of evidence quality and recommendations was made according to the Grading of Recommendations Assessment, Development and Evaluation Working Group.

Variability in triage practices for critically ill cancer patients: A randomized controlled trial.

Purpose: Intensive care triage practices and end-user interpretation of triage guidelines have rarely been assessed. We evaluated agreement between providers on the prioritization of patients for ICU admission using different triage guidelines.

Materials And Methods: A multi-centered randomized study on providers from 18 different countries was conducted using clinical vignettes of oncological patients.

Osteochondral Lesions of the Talus Treatment With Fresh Frozen Allograft.

Introduction: Symptomatic talar osteochondral lesions are about 50% refractory to conservative treatment requiring a surgical solution. In the case of large chronic lesions, the use of bone graft taken from tissue bank is an alternative that enables to fill the defect without causing donor site morbidity.

Material And Methods: Eight patients treated with talar osteochondral allograft in lesions greater than 20mm in diameter were analyzed - 4 males and 4 females aging 39.

[Calciphylaxis].

Calciphylaxis is vasculopathy characterized by ischemia and painful skin necrosis due to calcification and intimal fibroplasia of thrombosis of the panicular arterioles. It most frequently compromises patients with terminal chronic renal failure and has a high mortality rate. Biopsy of skin lesions is used as a diagnostic method.

Minimally Invasive Medial Plating of Low-Energy Lisfranc Injuries: Preliminary Experience with Five Cases.

Fracture dislocations involving the Lisfranc joint are rare; they represent only 0.2% of all the fractures. There is no consensus about the surgical management of these lesions in the medical literature.

Minimally invasive Akin osteotomy and lateral release: Anatomical structures at risk-A cadaveric study.

Background: Among the many surgical techniques used for hallux valgus correction, different osteotomies may be performed in the proximal phalanx as well as lateral release as associated procedures. The aim of this study is to analyze the anatomical relationships and the risks for the soft tissue lessions when performing the dorsomedial minimally invasive surgery (MIS) portal for the Akin osteotomy, and the MIS dorsolateral portal for lateral release, in order to define a safety zone when conducting the procedure in order to avoid complications.

Materials And Methods: The procedures were carried out on 16 fresh-frozen cadaveric feet.

Charcot neuroarthropathy in simultaneous kidney-pancreas transplantation: report of two cases.

Charcot neuroarthropathy (CN) is considered a major complication in diabetes mellitus (DM), and it is estimated that 1% of diabetic patients may develop this complication. Simultaneous kidney-pancreas transplantation (SKPT) is one of the most effective therapies for patients with type 1 DM and end-stage diabetic nephropathy. Some cases with a Charcot-modified clinical presentation during the postoperative convalescence period after SKPT have been described.

Clinical practice guidelines for evidence-based management of sedoanalgesia in critically ill adult patients.

Introduction: Optimal management of sedation, analgesia and delirium offers comfort and security for the critical care patient, allows support measures to be applied more easily and enables an integral approach of medical care, at the same time that lowers the incidence of complications, wich translates in better patient outcomes.

Objective: To update the Guía de práctica clínica basada en la evidencia para el manejo de la sedoanalgesia en el paciente adulto críticamente enfermo published in Medicina Intensiva in 2007, and give recommendations for the management of sedation, analgesia, and delirium.

Methodology: A group of 21 intensivists from 9 countries of the Federación Panamericana e Ibérica de Sociedades de Medicina Crítica y Terapia Intensiva, 3 of them also specialists in clinical epidemiology and methodology, gathered for the development of guidelines.

Delirium epidemiology in critical care (DECCA): an international study.

Introduction: Delirium is a frequent source of morbidity in intensive care units (ICUs). Most data on its epidemiology is from single-center studies. Our aim was to conduct a multicenter study to evaluate the epidemiology of delirium in the ICU.

[Latin American survey on delirium in critical patients].

Objectives: This study has aimed to establish the intensivist physician's concepts and practices in this region regarding the diagnosis, management and prevention of delirium in intensive care units (ICU).

Design: A survey was distributed among the FEPIMCTI member societies for distribution among its medical members.

Results: Eight hundred fifty-four intensive care physicians from 12 Latin America countries, most of them from Argentina, Mexico, Chile and Colombia, responded to the survey.

Role of an ABI3 homologue in dormancy maintenance of yellow-cedar seeds and in the activation of storage protein and Em gene promoters.

ABI3/VP1 proteins are members of a large group of transcription factors that act as intermediaries in regulating abscisic acid (ABA)-responsive genes during seed development, including those involved in reserve deposition, acquisition of desiccation tolerance and dormancy induction. CnABI3, an ABI3/VP1 gene homologue was recently cloned from yellow cedar, a conifer species that produces seeds that are deeply dormant at maturity. Here, we investigated whether the conifer ABI3/VP1 gene homologue shares characteristics with its angiosperm counterparts.

Assessment of neutron dosemeters around standard sources and nuclear fissile objects.

In order to evaluate the neutron doses around nuclear fissile objects, a comparative study has been made on several neutron dosemeters: bubble dosemeters, etched-track detectors (CR-39) and 3He-filled proportional counters used as dose-rate meters. The measurements were made on the ambient and the personal dose equivalents H*(10) and Hp(10). Results showed that several bubble dosemeters should have been used due to a low reproducibility in the measurements.
