The patient's complex medical history, including cardiac procedures and recent antibiotic use, added layers to the diagnostic challenge. Despite initial empirical antibiotic therapy, persistent fever prompted further investigations, leading to the identification of vegetation causing right ventricular outflow tract obstruction.
