Introduction: In the Indian subcontinent where tobacco chewing is rampant, composite 'bite' defects are common after oral cancer ablation, which comprise buccal mucosa, maxilla and mandible with or without skin. These are often reconstructed with soft tissue flaps like anterolateral thigh flaps, which provide ample soft tissue replacement. A common problem is palatal dehiscence where these flaps are inset, due to the dynamics of speaking and chewing, as well as the gravitational pull on the flap.

Article Synopsis
  • The paper discusses the use of free fibula flaps for reconstructing the maxilla and mandible after surgical removal, focusing on a method that utilizes virtual surgical planning and stereolithographic models to improve outcomes.
  • It highlights how virtual surgical planning has transformed complex reconstructions, especially for secondary procedures, while emphasizing the need for collaboration between surgeons and design/manufacturing teams.
  • The study details a series of 22 cases where this approach led to reduced surgical time and enhanced predictability of results, ultimately improving patients' quality of life.

Vascularized composite allotransplantation (VCA) has added another step to the reconstructive ladder, leading to a paradigm shift in the approach toward management of cases of upper limb amputations. In this article, we discuss in detail the technical aspects of proximal forearm level transplantation, as well as the immediate posttransplant monitoring and immunosuppression protocols. A 24-year-old male victim of an electric injury presented with a bilateral proximal forearm level amputation.
