Background: Vertical sleeve gastrectomy (VSG) has been associated with gastroesophageal reflux symptoms and, in adults, with a development of Barrett's esophagus. Adults with Barrett's esophagus identified at baseline before bariatric operation are generally advised against VSG operations. The role of preoperative esophagogastroduodenoscopy (EGD) in adolescents preparing for bariatric surgery is not clearly defined.

Extended periods of inactivity cause severe bone loss and concomitant deterioration of the musculoskeletal system. Considerable research has been aimed at better understanding the mechanisms and consequences of bone loss due to unloading and the associated effects on strength and fracture risk. One factor that has not been studied extensively but is of great interest, particularly for human spaceflight, is how multiple or repeated exposures to unloading and reloading affect the skeleton.

Article Synopsis
  • The study focuses on optimizing two molecular fragments, 2-amino-3-benzyloxypyridine and 3-(2-(4-pyridyl)ethyl)indole, for their effectiveness against p38alpha MAP kinase, revealing their initial IC(50) values.
  • Through case studies, the authors explore synthesized compounds, structure-activity relationships, and binding modes, leading to the development of two potent lead series.
  • The optimization process involved expanding fragments into nearby pockets and combining overlapping fragments, utilizing the mobile activation loop for improving both potency and selectivity of the kinase inhibitors.
