Interlocking screw backout is a significant complication of femoral nailing that may lead to pain, fixation failure, and need for reoperation. The risk for this is increased in osteoporotic bone or in cases of so-called "extreme nailing" when the articular segment is short, and fixation is limited. A newly developed retrograde femoral nailing system (DePuy Synthes RFN-Advanced™ Retrograde Femoral Nailing System (DePuy Synthes, Monument, CO)) was designed specifically to prevent this known failure mode and was recently approved for use within the United States.

It has been shown that many molecules released by adipose tissue (AT) into interstitial fluid can reach the bloodstream preferentially via lymphatic system. Worsened lymphatic drainage may alter interstitial fluid (ISF) composition and thus affect microenvironment of adipocytes. Nevertheless, the effect of lymphatic drainage on AT functions remains unknown.

Background: Historically, there has been excellent survivorship of total hip arthroplasty with newer implants expected to outperform prior generations. Review of our institutional database identified problems with one particular cross-linked polyethylene liner in mid-term follow-up, which has not been reported in the literature. We report on the technical aspects of this liner, as well as the clinical and radiographic factors associated with early failure of this bearing in our practice.
