Purpose: To improve work ability and health-related quality of life (HRQOL) cancer patients were offered a "Rapid-Return to Work" program. However, several patients did not improve their work status after completing the program. This implied the termination of centralized care, and integration of these individuals into the local communities. Method: A double-blind crossover study (2 x 8 weeks; n = 34), interrupted by a two-week single-blind washout period, was employed. Assessments included the Schedule for Handicaps, Behaviour, and Skills (SHBS) and Clinical Global Impression (CGI).
The Norwegian Government has decided to reform the care of the mentally retarded by placing the responsibility for medical and social care with the local authorities. From 1 January 1991 the mentally retarded are to be integrated into the community. Persons with Down syndrome constitute 20-30% of the mentally retarded in institutions.

Plasma concentrations of TSH, PTH, LH, FSH, ACTH, glucagon and insulin all working through cell membrane receptors were elevated. A generalized cell membrane defect was suggested to be the pathophysiological abnormality in these patients.
