Hyperbaric oxygen treatment for paracentral acute middle maculopathy: a case study.

A 47-year-old active-duty Navy diver with a complicated past medical history which resulted in his designation as not physically qualified (NPQ) for diving duty in 2016 presented on 07 May 2021 complaining of left-sided blurred vision. On exam by the attending undersea medical officer, he was found to have a left upper inner and upper outer quadrant visual field defect and a central scotoma. Urgent referral to ophthalmology ruled out retinal detachment but resulted in an initial diagnosis of a left branch retinal artery occlusion without embolus.

Enlarged and echogenic kidneys while on a pediatric ventricular assist device.

Pediatric ventricular assist devices (VAD) specially designed for small children have been increasingly implanted in North America. We present unexpected renal findings developing in three children receiving circulatory support using VAD (EXCOR Pediatric Ventricular Assist Device; Berlin Heart Inc., The Woodlands, TX).

Long-term safety of dichloroacetate in congenital lactic acidosis.

We followed 8 patients (4 males) with biochemically and/or molecular genetically proven deficiencies of the E1α subunit of the pyruvate dehydrogenase complex (PDC; 3 patients) or respiratory chain complexes I (1 patient), IV (3 patients) or I+IV (1 patient) who received oral dichloroacetate (DCA; 12.5 mg/kg/12 h) for 9.7 to 16.

A patient with an unusual cause right lower quadrant pain and vomiting: pyelonephritis of an ectopic right kidney masquerading as acute appendicitis.

An adolescent female presented with one day of abdominal pain and clinical findings of acute appendicitis. CT scan revealed an ectopic right kidney with changes of acute pyelonephritis. This paper underscores the importance of imaging the right pelvis prior to surgical intervention in suspected cases of acute appendicitis in children.

Leflunomide therapy for BK virus allograft nephropathy in pediatric and young adult kidney transplant recipients.

BKVAN affects about 5% of kidney transplant recipients and may lead to graft failure. Treatment for BKVAN is challenging. Leflunomide, an immunosuppressant with antiviral activity in vitro was used successfully in some adult patients but there are no reports of its use in pediatric patients.
