Metal on metal hip (MoM) resurfacing / replacement has recently been one of the most discussed topics in orthopaedics. Significant revision rates and complications are associated with MoM. We report a patient presenting with infection of a total hip prosthesis in conjunction with a psoas abscess in which the presentation is complicated by radiological evidence of suspected metallic debris in the lower limb tissues and elevated chromium and cobalt levels, suggestive of metallosis secondary to MoM.

A new prenylated flavonol, petalopurpurenol (1), and a known dihydroflavonol, petalostemumol (2), have been isolated by DNA scission-guided fractionation of the organic portion of a 20% MeOH/CHCl3/H2O partition of a 50% MeOH/CHCl3 extract of the roots of Petalostemon purpureus. Compound 2 displayed moderate activity in DNA-scission assay. Both compounds 1 and 2 were evaluated for cytotoxicity in a panel of human cancer cell lines.

The structures of the compounds were elucidated using a variety of spectroscopic techniques. The structure of 1 was shown to be 7,4'-dimethoxy-8-hydroxyhomoisoflavone, and 2 was shown to be 4'-methoxy-7,8-dihydroxyhomoisoflavone.

Compound 1 has the structure 5'-(alpha, alpha-dimethylallyl)-5,7,2',4'-tetrahydroxyisoflavone; compound 2 is similar but also contains the 3'-(gamma,gamma-dimethylallyl) substituent. The alpha,alpha-dimethylallyl substituent is rarely observed, and the combination of the alpha,alpha- and gamma,gamma-dimethylallyl substituents is unprecedented.
