A middle-aged gentleman, presented to our outpatient department with painful skin lesions suggestive of disseminated herpes zoster. Further examination revealed bilateral cerebellar signs. He had a history of receiving a third dose of AZD1222 vaccine fourteen days prior to the onset of skin lesions but had no other significant medical history.

Exosomes, which are nano-sized transport bio-vehicles, play a pivotal role in maintaining homeostasis by exchanging genetic or metabolic information between different cells. Exosomes can also play a vital role in transferring virulent factors between the host and parasite, thereby regulating host gene expression and the immune interphase. The association of inflammation with disease development and the potential of exosomes to enhance or mitigate inflammatory pathways support the notion that exosomes have the potential to alter the course of a disease.

Background: Community pharmacists' usually are not able to be actively involved in the identification and referral of presumptive TB cases approaching their pharmacy for over-the-counter (OTC) drugs. The main barriers from their perspective were increased patient volume and work burden. Therefore, a new methodology was adopted by appointing an auxiliary pharmacist who will be actively involved in the identification and referral of presumptive TB cases.
