This article introduces the Generalized Analog Regression Downscaling method Large Ensemble (GARD-LENS) dataset, comprised of daily precipitation, mean temperature, and temperature range over the Contiguous U.S., Alaska, and Hawaii at 12-km, 4-km, and 1-km resolutions, respectively.

Can a practicing surgeon detect early lymphedema reliably?

Background: Lymphedema may be identified by simpler circumference changes as compared with changes in limb volume.

Methods: Ninety breast cancer patients were prospectively enrolled in an academic trial, and seven upper extremity circumferences were measured quarterly for 3 years. A 10% volume increase or greater than 1 cm increase in arm circumference identified lymphedema with verification by a lymphedema specialist.

Transient left ventricular dysfunction in childhood sickle cell disease.

For unclear reasons, myocardial infarction is rare in childhood sickle cell disease, whereas lung, bone, and brain infarcts are more common. During vasoocclusive crisis and infection, acute myocardial ischemia and chronic volume overload from anemia may result in myocardial dysfunction. We report a child who had reversible cardiac dysfunction that mimicked myocardial infarction.

Diabetic nephropathy: natural course, survivorship and therapy.

Diabetes mellitus is a disease with major social and economic consequences. End-stage renal disease (ESRD) due to diabetes occurs in as many as 30% of patients with juvenile diabetes, providing a large percentage (up to 33%) of all patients in need of therapy for ESRD. We have reviewed the natural course of the nephropathy in type I diabetes mellitus and the results of dialysis and transplantation therapy with particular regard to survival and morbidity.
