Background: Anti-Müllerian hormone (AMH) is a widely used marker for estimating ovarian reserve, and it may predict response to ovarian stimulation. While AMH is considered a stable, cycle-independent marker, studies have shown it can exhibit significant fluctuations based on factors like age, reproductive stage, and menstrual cycle phase. The fluctuations in AMH levels can make it challenging to predict individual responses accurately, particularly when the AMH is not measured in the COS cycle.

Article Synopsis
  • The study assessed how adding letrozole during ovarian stimulation affects treatment outcomes in women in POSEIDON groups 3 and 4.
  • A total of 449 patients were analyzed, with no significant difference in live birth rates between those who received letrozole and those who did not (11% vs. 9%).
  • Letrozole reduced the total gonadotropin dosage, shortened stimulation duration, and lowered peak estradiol levels, suggesting potential cost savings despite no impact on live births.

Objective: Despite recent advancements in assisted reproductive technology (ART), the effective management of patients with poor ovarian response (POR) remains a formidable challenge. While various treatment strategies and predictors of live births have been documented to provide guidance to fertility specialists in managing poor responders, research efforts have predominantly encompassed all POSEIDON groups. In this study, our objective was to analyze the factors correlated with live births (LB) within a subset of the POSEIDON groups, with a particular focus on POSEIDON groups 3 and 4.

Objective: Endometrioma surgery is associated with a reduction in ovarian reserve. However, removal of an endometrioma may increase the likelihood of a spontaneous conception. The objective of this study was to assess the pre-operative and operative variables affecting spontaneous conception following endometrioma surgery.
