Supernumerary ribs are a well-documented congenital anomaly that can occur at any point of the vertebral column, most commonly in the cervical or lumbar region. However, accessory ribs found in the sacrococcygeal region are exceptionally rare and may be difficult to distinguish from other bony manifestations of the pelvic girdle. During cadaveric dissection, a pair of sacral "ribs" were found projecting from the left posterolateral sacral region.

On the basis of evidence that p53 mutation is involved in the development of human neoplasia, we examined the prognostic value of p53 alterations in combination with neu/erbB-2 amplification.
Patients And Methods: A consecutive series of women were observed for recurrence and death (median follow-up of 85 months) and tumors from 543 individuals were analyzed for p53 mutation status and neu/erbB-2 amplification.
Purpose: It remains a challenge to predict which women with axillary node-negative (ANN) breast cancer at greatest risk of relapse may benefit most from adjuvant therapy. Increases in neu/erbB-2 have been implicated in breast cancer prognosis. Although overexpression has been investigated extensively, this study represents the first prospective assessment of the prognostic value of neu/erbB-2 DNA amplification in a cohort of women with newly diagnosed ANN.
View Article and Find Full Text PDFLipids and lipoproteins have been associated with breast cancer risk; however, published results have been inconsistent. To clarify these associations, we measured fasting lipids in women undergoing breast biopsies. A case-control study examined the association of fasting levels of lipids with histologically defined breast cancer risk.

The aim of this study was to define histopathologic parameters that can be used for clinically relevant treatment decisions.

The clinical and pathologic features of four cases of intussusception of the appendix are reported and the literature is reviewed. All patients had vague abdominal symptoms. The diagnosis of intussusception of the appendix was not made preoperatively in any of these cases.

Illness was associated with use of sigmoidoscopes cleaned by one clinic assistant and appeared to be caused by 2% glutaraldehyde disinfectant solution left in the instruments after cleaning.
Animal Studies: In subsequent animal studies, colonic instillation of 2% glutaraldehyde solutions caused bloody diarrhea and a distinctive pattern of mucosal damage; similar changes were seen in a review of pathologic samples from other human cases of glutaraldehyde disinfectant-associated diarrhea.
Cystic glandular polyps of the stomach, histologically distinct from hyperplastic and adenomatous polyps, may undergo spontaneous regression. We report their occurrence in a 50-year-old woman with adult celiac disease, an association not previously described. They regressed incompletely when the patient was given a gluten-free diet.

Composite glandular-carcinoid tumors of the large bowel are rare. We describe two cases that appeared to be at a relatively early stage in their development. Each of these cases was a composite colorectal adenoma-carcinoid--an entity that has not previously been described.

The phenotypes expressed were comparable to those of normal salivary gland cells. In the normal glands, myoepithelial cells were immunoreactive for glial fibrillary acidic protein (GFAP), S-100 protein, and keratin; acinic cells exhibited strong, predominantly nuclear S-100 staining and weaker keratin staining; intercalated ducts had both cytoplasmic and nuclear S-100 positivity; and several epithelial antigens were observed throughout the ductal system.

Complications that may arise in these patients include acute hemorrhage and malignant transformation. With appropriate dietary therapy, they may partially or completely regress.
View Article and Find Full Text PDFFamilial adenomatous polyposis coli is now known to encompass a more complicated spectrum of anomalies than was initially suspected. The term Gardner's syndrome, broadened from its original definition, currently includes all cases of familial polyposis coli with extracolonic neoplasms. Small-bowel adenomata occur in approximately 12% of these patients, and sporadic instances of biliary neoplasia have also been reported.

Hepar lobatum is now a rarity in developed countries. In this article, we describe three cases with typical liver morphology associated with carcinoma of the breast metastatic to the liver, treated with combination chemotherapy. The pathogenesis of hepar lobatum in our cases is multifactorial with tumor-related desmoplasia and effects of chemotherapy, with resultant tumor necrosis and fibrosis playing a major role.

The sites of most frequent occurrence are the ocular adnexae, where sebaceous carcinoma presents as an infiltrating malignancy with a poor prognosis. Cutaneous sebaceous carcinoma is rare at other sites but has a better prognosis.

The malignant cells were arranged in monocellular layers, pseudorosettes and in perivascular palisades in a fibrillar background. The nuclei were monomorphous with small nucleoli and finely granular chromatin.

The physaliferous cells in signet-ring, pearl-like formations and the trabecular arrangement with rounded contours are distinctive. Other cell types and cellular arrangements are also described.

The clinical data, the gross and microscopic pathological features, and the histochemical mucin profile of these two lesions are described in detail.
