Purpose: Fixation of metacarpal or phalanx bone fractures is usually performed under general anesthesia (GA) or regional anesthesia and with the use of a tourniquet to minimize bleeding. However, the use of tourniquet causes pain and discomfort after surgery. Wide-awake local anesthesia no tourniquet (WALANT) enables the surgery to be performed with the patient fully awake and without a tourniquet, which allows intraoperative assessment of function during surgery.

Article Synopsis
  • This study investigates how apical periodontitis (AP) affects glycated hemoglobin (HbA1c) and inflammatory markers in individuals with Type 2 diabetes and healthy controls.
  • A total of 280 individuals were assessed, including 140 with diabetes (70 with AP and 70 without AP) and 140 healthy participants (70 with AP and 70 without AP), using radiologic examinations and blood tests for various inflammatory markers and HbA1c levels.
  • Results indicated that individuals with AP had higher levels of inflammatory markers and HbA1c in both groups, suggesting that AP and the size of the lesion may worsen glycemic control and increase systemic inflammation in Type 2 diabetes patients.*

Genetic signatures have added a molecular dimension to prognostics and therapeutic decision-making. However, tumour heterogeneity in prostate cancer and current sampling methods could confound accurate assessment. Based on previously published spatial transcriptomic data from multifocal prostate cancer, we created virtual biopsy models that mimic conventional biopsy placement and core size.

The optimal management of hypertension in individuals aged 80 years or older with frailty remains uncertain due to multiple gaps in evidence. Complex health issues, polypharmacy, and limited physiological reserve make responding to antihypertensive treatments unpredictable. Patients in this age group may have limited life expectancy, so their quality of life should be prioritized when making treatment decisions.
