To determine the part played by altered gastric emptying in the modification of glucose absorption by gel fibres, glucose tolerance tests were done in seven healthy volunteers with and without the addition of pectin to the ingested glucose solution and after pharmacological inhibition of gastric emptying with propantheline. Compared with the controls, pectin significantly reduced blood-glucose. Propantheline had a similar but more pronounced effect.

Methods for the estimation of plasma paracetamol which depend on acid hydrolysis to p-aminophenol without a prior extraction step also measure inactive metabolites which are present in high concentrations. The extent of the overestimate obtained with such methods was determined using 24 samples from patents after paracetamol overdosage. There was a positive error of between 40 and 700% compared with a high-performance liquid chromatography reference method which measured only unchanged paracetamol.

In a double-blind study involving 165 patients we examined the role of mexiletine, a new antiarrhythmic drug, for the prophylaxis of ventricular arrhythmias after acute myocardial infarction. Mexiletine or placebo was given orally to patients on arrival in the coronary care unit, and continuous electrocardiographic tape recordings were used to document arrhythmias. Ventricular tachycardia and R-on-T ventricular ectopic beats were significantly reduced in the mexiletine patients, but too few episodes of ventricular fibrillation occurred for statistical comment.

1 Self-poisoning with a selective weedkiller containing 2,4-D and mecoprop in a 39 year old man resulted in prolonged deep coma, pyrexia, hyperventilation, hypoxaemia, myotonia, skeletal muscle damage and electrocardiographic changes consistent with cardiomyopathy. 2 The admission plasma concentrations of 2,4-D and mecoprop were 400 and 751 microgram/ml respectively. The patient remained gravely ill with no signs of improvement for 2 days with supportive therapy and there was no fall in the 2,4-D level.

Eight healthy male volunteers ingested an aqueous solution containing acetaminophen (20 mg/kg) and a nonabsorbable isotopic marker. The concentrations of unconjugated acetaminophen in samples of blood plasma taken at frequent intervals were measured by gas-liquid chromatography. The data points followed a smooth curve in most cases and were fitted to the classical two-compartment pharmacokinetic model to obtain KA, the apparent first-order rate constant for absorption from the gastrointestinal tract.

Flunitrazepam or diazepam with atropine and a combination of phenoperidine, droperidol and cyclizine (neuroleptanalgesia) were compared as premedication in three groups of 25 patients undergoing routine upper gastrointestinal endoscopy. Drug doses were titrated carefully against response and all three regimes were found to be similar in terms of safety, patient co-operation, relaxation and speed of recovery. Neuroleptanalgesia however produced a statistically significant rise in endtidal pCO2 and systolic blood pressure.

Using the rate of absorption of paracetamol following oral administration of the drug, gastric emptying was measured in 21 patients following hysterectomy. Gastric emptying was inhibited markedly in patients receiving narcotic analgesia after operation, but only a moderate delay was observed in patients undergoing extradural analgesia.

Cultures of rat cerebellum grown on glass coverslips coated first with collodion and then collagen are fixed in glutaraldehyde to which ruthenium red has been added. After fixation, the collodion comes off the glass coverslips and the 3 layers: collodion-collagen-culture are handled as one piece. Following immersion in glycerol, selected areas are cut from the piece; the excised areas are a little smaller than those of the specimen carriers used in a Balzers apparatus.

Fifteen patients with paracetamol (acetaminophen) poisoning were treated with intravenous N-acetylcystein (300 mg/kg given over 20 h). Mean admission and 4 h plasma-paracetamol concentrations were 262 and 369 microgram/ml, respectively. Liver-function tests remained normal or were only slightly disturbed in 11 of 12 patients treated within 10 h of paracetamol ingestion.

The plasma half lives of antipyrine, paracetamol, and lignocaine given by mouth were measured in 23 patients with stable chronic liver diseases of varying severity. Fifteen patients received all three drugs and 19 at least two. The half life of paracetamol was abnormally prolonged in nine out of 17 patients (mean 2-9 hours, normal 2-0 hours), of antipyrine in 10 out of 19 patients (mean 30-4 hours, normal 12-0 hours), and of lignocaine in 19 out of 21 patients (mean 6-6 hours, normal 1-4 hours).
