Objectives: Evidence indicates that, in pediatric patients with parenteral nutrition-associated cholestasis (PNAC), the use of a 100% fish oil lipid emulsion (FOLE) increased the likelihood of PNAC resolution and reduced the likelihood of liver transplantation compared with a 100% soybean oil lipid emulsion (SOLE). To evaluate the potential economic benefit, we conducted a cost-effectiveness analysis comparing FOLE with SOLE.

Study Design: A discrete event simulation model evaluated cost-effectiveness by simulating clinical outcomes and estimating associated healthcare costs in pediatric patients with PNAC receiving parenteral nutrition (PN) with FOLE (1 g/kg) or SOLE (1.

Article Synopsis
  • Hospitalized patients with acute decompensation of cirrhosis are at high risk for progressing to acute-on-chronic liver failure, which significantly increases mortality rates; this study aimed to find key predictors of this progression.
  • The research spanned two years and included 625 patients across India, dividing them into a derivation-cohort and a validation-cohort to develop and compare predictive models against established scores like MELD3.0.
  • Findings showed that about 32.2% of patients progressed to ACLF, with higher bilirubin and leukocyte counts among those affected, and a new PRE-ACLF Model was more effective in predicting ACLF risk than existing scores.

Background: The growth hormone-insulin-like growth factor (GH-IGF-1) axis and its impairment with sarcopenia, frailty, bone health, complications, and prognosis are not well characterized in cirrhosis.

Methods: We investigated the adult decompensated cirrhosis out-patients at a tertiary care institute between 2021 and 2023 for serum GH and IGF-1 levels, and associated them with sarcopenia (CT-SMI in cm/m), liver frailty index (LFI), osteodystrophy (DEXA), clinical decompensations (overall, ascites, encephalopathy, infection, and bleed), and survival up to 180 days.

Results: One-hundred-seventy-two patients, 95% males, aged 46.

Background: The current definition of lean is based on body mass index (BMI). However, BMI is an imperfect surrogate for adiposity and provides no information on central obesity (CO). Hence, we explored the differences in clinical profile and liver disease severity in lean patients with nonalcoholic fatty liver disease (NAFLD) with and without CO.

Background & Aims: Chronic hepatitis C-related decompensated cirrhosis is associated with lower sustained virologic response (SVR)-12 rates and variable regression of disease severity after direct-acting antiviral agents. We assessed rates of SVR-12, recompensation (Baveno VII criteria), and survival in such patients.

Methods: Between July 2018 and July 2023, patients with decompensated chronic hepatitis C-related cirrhosis after direct-acting antiviral agents treatment were evaluated for SVR-12 and then had 6-monthly follow-up.

Globally, nearly half of deaths from cirrhosis and chronic liver diseases (CLD) and three-quarters of deaths from hepatocellular carcinoma (HCC) occur in the Asia-Pacific region. Chronic hepatitis B is responsible for the vast majority of liver-related deaths in the region. Metabolic dysfunction-associated steatotic liver disease (MASLD) is the most common form of CLD, affecting an estimated 30% of the adult population.

Article Synopsis
  • Acute liver failure (ALF) is a critical condition that may require liver transplantation, with its main causes in India being viral, particularly hepatitis A which accounts for 44.2% of cases.
  • A study analyzed 183 ALF patients from four major hospitals in India between 2021 and 2023, highlighting the effectiveness of interventions such as plasma exchange (PLEx) and continuous renal replacement therapy (CRRT), showing a low rate of liver transplants at only 3.8%.
  • Key predictors of mortality included advanced hepatic encephalopathy and meeting specific medical criteria at admission, suggesting the importance of early intervention and potential for improved outcomes with vaccination against hepatitis A.

Article Synopsis
  • The study aimed to evaluate short-term outcomes such as mortality and length of stay for extremely low birth weight infants with spontaneous intestinal perforation, comparing three surgical approaches: peritoneal drain (PD), laparotomy after PD (PD-LAP), and straight laparotomy (LAP).
  • Out of 729 identified infants, those treated with PD had lower gestational ages and higher infection rates, with PD associated with higher mortality rates compared to PD-LAP and LAP in initial analysis, but this link was not confirmed in more complex statistical models.
  • Ultimately, the findings suggest that the type of initial surgical approach does not significantly affect mortality or length of stay for these infants, indicating that other factors such

Hospitalized patients with cirrhosis frequently require critical care management for sepsis, HE, respiratory failure, acute variceal bleeding, acute kidney injury (AKI), shock, and optimization for liver transplantation, while outpatients have unique care considerations. Point-of-care ultrasonography (POCUS) enhances bedside examination of the hepatobiliary system and relevant extrahepatic sites. POCUS includes cardiac US and is used to assess volume status and hemodynamic parameters like cardiac output, systemic vascular resistance, cardiac contractility, and pulmonary artery pressure, which aid in the early and accurate diagnosis of heart failure, cirrhotic cardiomyopathy, porto-pulmonary hypertension, hepatopulmonary syndrome, arrhythmia, and pulmonary embolism.

Background: The increase in prevalence of liver disease globally will lead to a substantial incremental burden on intensive care requirements. While liver transplantation offers a potential life-saving intervention, not all patients are eligible due to limitations such as organ availability, resource constraints, ongoing sepsis or multiple organ failures. Consequently, the focus of critical care of patients with advanced and decompensated cirrhosis turns to liver-centric intensive care protocols, to mitigate the high mortality in such patients.

Non-cirrhotic portal hypertension (NCPH) is a well-recognized clinico-pathological entity, which is associated with clinical signs and symptoms, imaging, and endoscopic features of portal hypertension (PHT), in absence of cirrhosis. In patients with NCPH without known risk factors of PHT or extrahepatic portal vein thrombosis, the condition is called idiopathic non-cirrhotic portal hypertension (INCPH). There are multiple infectious, immune related causes, systemic diseases, drug and toxin exposures, haematological disorders, and metabolic risk factors that have been associated with this INCPH.
