Article Synopsis
  • The study focuses on improving residents' training in managing breech deliveries, a critical skill in obstetrics, especially as vaginal deliveries of breech cases decline in favor of cesarean sections.
  • A randomized controlled trial was conducted where obstetrics residents were divided into two groups: one received a formal lecture while the other used digital materials for home learning, with both groups evaluated through a simulation test.
  • Results showed no significant differences in performance based on the type of learning, although the digital group scored slightly higher in some questionnaire aspects, indicating that both learning methods are effective in training for vaginal breech deliveries.

Background: Fetal growth restriction is associated with perinatal morbidity and mortality. Early identification of women having at-risk fetuses can reduce perinatal adverse outcomes.

Objectives: To assess the predictive performance of existing models predicting fetal growth restriction and birthweight, and if needed, to develop and validate new multivariable models using individual participant data.

Objective: To predict birth weight at various potential gestational ages of delivery based on data routinely available at the first antenatal visit.

Design: Individual participant data meta-analysis.

Data Sources: Individual participant data of four cohorts (237 228 pregnancies) from the International Prediction of Pregnancy Complications (IPPIC) network dataset.

Article Synopsis
  • MV-Flow is a tool designed to gather detailed data on slow-moving blood flow.
  • It provides improved visualization of the vein of Galen's aneurysmal malformation, including its structure and related blood vessels.
  • This technique surpasses traditional ultrasound methods in accuracy and detail for such vascular assessments.

Objective: The Moon has a noticeable influence on the Earth due to its gravity, the most visible manifestation of which are tides. We aimed to see if the Moon's daily cycle, like the Sun's, affects the prevalence and incidence of childbirth.

Methods: In this retrospective cohort study, we examined all deliveries at the Academic Hospital of Udine between 2001 and 2019.

Congenital anomalies of the umbilical cord are associated with an increased risk of pregnancy and perinatal complications. Some anomalies of the cord have a higher prevalence than other fetal structural anomalies. The most common anomalies are the absence of an umbilical artery and velamentous insertion of the cord (with or without vasa previa).

Objectives: To compare characteristics of labor, cardiotocography traces, and maternal and neonatal outcomes, in a cohort of pregnancies at term complicated by maternal intrapartum pyrexia, with or without a histologic diagnosis of chorioamnionitis.

Methods: This is a retrospective case-control study including pregnancies at term with detection of maternal intrapartum pyrexia, delivered between January 2020 and June 2021. Cardiotocography traces were entirely evaluated, since admission till delivery, and classified according to the International Federation of Obstetrics and Gynecology (FIGO) guideline.

Background: There are limited data to guide the diagnosis and management of vasa previa. Currently, what is known is largely based on case reports or series and cohort studies.

Objective: This study aimed to systematically collect and classify expert opinions and achieve consensus on the diagnosis and clinical management of vasa previa using focus group discussions and a Delphi technique.

Article Synopsis
  • In early 2023, a study demonstrated that vaccinating pregnant women effectively reduces the chances of severe COVID-19 complications and maternal health issues.
  • The INTERCOVID-2022 study, conducted across 40 hospitals in 18 countries, analyzed how COVID-19 during pregnancy affects newborns and the benefits of maternal vaccination during the Omicron variant period.
  • Results showed that newborns from mothers who received a booster vaccine had significantly lower risks of contracting COVID-19 and experiencing preterm birth compared to those from unvaccinated mothers.

Article Synopsis
  • The Italian Association of Preeclampsia (AIPE) and the Italian Society of Perinatal Medicine (SIMP) collaborated to explore maternal hemodynamics and its implications for pregnancy outcomes.
  • Experts reviewed existing literature and proposed recommendations on managing maternal cardiovascular profiles, leading to a classification system for pregnancy-related hypertension and complications based on hemodynamic states.
  • The findings suggest that understanding these hemodynamic profiles can enhance clinical decision-making, despite the need for more extensive studies to solidify the evidence.

Objective: The study aims to assess how oral misoprostol for cervical ripening affects the time of cesarean delivery (CD) for fetal heart rate (FHR) abnormalities in pre-eclampsia patients. Secondary goals include determining the role of uterine hyperstimulation, comparing misoprostol with Foley catheter, and identifying risk factors for FHR abnormalities associated with CD.

Methods: A previously published randomized clinical trial was subjected to a secondary analysis (NCT01801410).

This practice guideline follows the mission of the World Association of Perinatal Medicine in collaboration with the Perinatal Medicine Foundation, bringing together groups and individuals throughout the world, with the goal of improving the management of preterm labor. In fact, this document provides further guidance for healthcare practitioners on the appropriate use of examinations with the aim to improve the accuracy in diagnosing preterm labor and allow timely and appropriate administration of tocolytics, antenatal corticosteroids and magnesium sulphate and avoid unnecessary or excessive interventions. Therefore, it is not intended to establish a legal standard of care.

Article Synopsis
  • A cesarean scar pregnancy occurs when a gestational sac implants in the scar from a previous cesarean delivery, leading to significant risks for the mother.
  • The study aimed to analyze ultrasound findings and management strategies for cesarean scar pregnancies, utilizing an international registry that collected data from various healthcare centers.
  • Out of 460 patients analyzed, 58.7% received surgical treatment, 26.7% were managed medically, and the effectiveness of these interventions was evaluated based on successful resolution of the pregnancy.

Introduction: The aim of our study was to investigate the causes of fetal growth <10th centile diagnosed <26 weeks' gestation in singleton pregnancies and compare pregnancy outcomes in relation to the identified etiology.

Material And Methods: Historical cohort study conducted in two Italian hospitals which included all small-for-gestational-age fetuses diagnosed between 18+0 and 26+0 weeks over a 10-year period. Fetuses were divided into three groups depending on the prenatally suspected etiology: chromosomal abnormalities (Group 1), malformations (Group 2) and isolated (Group 3).

Introduction: Hepatocellular carcinoma (HCC) is the most frequent primary malignant liver tumor and typically develops in the context of chronic liver disease, such as liver cirrhosis or chronic hepatitis B virus infection. Ultrasound evaluation, CT scan, and MRI are used to detect HCC. α-fetoprotein (AFP) is a common marker used to detect HCC in the non-pregnant population, which notoriously increases in pregnant women in relation to gestational age.

Objective: Monochorionic (MC) triplet pregnancies are extremely rare and information on these pregnancies and their complications is limited. We aimed to investigate the risk of early and late pregnancy complications, perinatal outcome and the timing and methods of fetal intervention in these pregnancies.

Methods: This was a multicenter retrospective cohort study of MC triamniotic (TA) triplet pregnancies managed in 21 participating centers around the world from 2007 onwards.
