Unusual imaging features of urothelial cancer of the renal pelvicaliceal system can be challenging for any radiologist. These manifestations include noncalcified and calcified focal infiltrative parenchymal masses, incidentally detected tumors in a hydronephrotic kidney due to ureteropelvic junction obstruction, transpelvic infiltrating solid masses extending through the retroperitoneum, and tumors primarily invading the perirenal fat. Less common manifestations include massive papillary frond-like projections within a large hydronephrotic sac, large multiloculated cystic masses with thick and irregular septa, tumors invading the renal vein, and paraaortic lymph node metastases as the only sign of an undetectable primary tumor of the renal collecting system.

Antitumor necrosis factor alpha (anti-TNF-alpha) therapy in perianal Crohn's disease (CD) is widely established but recent studies suggest that the underlying fistula tract and inflammation may persist. Treatment with a monoclonal antibody against interleukin (IL)-12 was reported to induce clinical responses and remissions in patients with active CD. The aim of our study was to analyze the cytokine network (TNF-alpha, IL-12, IL-1beta, and IL-6) in 12 patients with chronic perianal CD and a Crohn's disease activity index (CDAI) score <150 to exclude active intestinal disease, in 7 patients with indeterminate colitis (IC) after restorative proctocolectomy with perianal complications, in 7 patients with active intestinal CD without perianal manifestations, and in 19 healthy controls.

Background & Aims: This study extends our previously reported observations that various immunological factors are associated with the occurrence of histologically proven recurrent hepatitis C. The two specific issues investigated were to confirm the associations of MHC alleles and donor/recipient mismatch with the occurrence of recurrent hepatitis C in an independent cohort of newly transplanted patients and to look for immunologic and nonimmunologic variables affecting the severity of the recurrent disease.

Unusual radiologic manifestations of a renal cell carcinoma (RCC) can present a diagnostic challenge. These manifestations include bilateral synchronous multifocal tumors, a small RCC with synchronous adrenal metastasis, and RCC associated with bulky abdominal lymphoma. Less common manifestations include multiseptated cystic carcinoma simulating a moderately complex renal cyst at ultrasonography (US), paraaortic metastatic adenopathy as the only sign of an undetectable primary renal neoplasm, RCC causing a large arteriovenous fistula, RCC simulating angiomyolipoma, and a nonfunctioning kidney due to transparenchymal renal propagation of cancer associated with a tumor thrombus occluding the renal vein.
