Purpose: This study evaluates the Social Return on Investment (SROI) of implementing measures to prevent fragility fractures in postmenopausal women with osteoporosis (OP) in Spain.

Methods: A group of 13 stakeholders identified necessary actions for improving refracture prevention and assessed the investment required from the Spanish National Health System (SNHS), considering direct, indirect, and intangible costs over a one-year period. Unitary costs were sourced from scientific literature and official data, and intangible costs were estimated through surveys on women's willingness to pay for better health-related quality of life.

Background: Transgender men face reproductive challenges due to the potential impact on fertility of gender-affirming hormone therapy (GAHT) and surgical interventions. Testosterone therapy during "female to male" transition leads to anovulation and amenorrhea. Although these effects are typically reversible upon stopping treatment, the long-term effects of androgens on future fertility and health of potential children remain poorly known.

Introduction: Respiratory syncytial virus (RSV) is the leading cause of acute lower respiratory infections (ALRI) in children under one year of age. In high-income countries, RSV infections cause a significant overload of care every winter, imposing a significant burden to the healthcare system, which has made the development of prevention strategies a major global health priority. In this context, a new bivalent RSV prefusion F protein-based vaccine (RSVpreF) has recently been approved.

Introduction: Atopic dermatitis (AD) is a chronic, inflammatory skin disease characterized by itchy, painful, and dry skin. Despite the great number of available therapies, economic evaluations are still needed to provide evidence on their cost efficiency. This research aimed to evaluate the cost effectiveness of the Janus kinase (JAK) inhibitor abrocitinib (200 mg) compared with dupilumab (300 mg), tralokinumab (300 mg), baricitinib (2 and 4 mg), and upadacitinib (15 and 30 mg) for the treatment of patients with severe AD from the Spanish National Health System (NHS) perspective.

Article Synopsis
  • The study focuses on male fertility preservation (FP) through sperm banking for men aged 15-49 diagnosed with testicular cancer (TC) or lymphomas (L) in France in 2018.
  • It analyzed data from the French National Cancer Institute and sperm banking centers, estimating sperm banking rates of 41% for TC, 40% for Hodgkin L, and 7% for non-Hodgkin L among diagnosed men.
  • The findings suggest a need for improved patient education and awareness to increase sperm banking rates and assess factors leading to the lack of sperm preservation before gonadotoxic treatments.

Article Synopsis
  • The study aimed to find patient characteristics that influence the success of retrieving immature oocytes during ovarian tissue cryopreservation for fertility preservation.
  • Researchers analyzed data from 257 patients over three years, focusing on factors like age, ovulatory status, and prior chemotherapy, which affected oocyte retrieval success rates.
  • The results indicated that patients with regular ovulatory cycles and without prior chemotherapy had higher success rates in retrieving oocytes, while certain conditions like Turner syndrome significantly decreased success chances.

Testicular germ cell tumours (TGCTs) are the most common tumours in young adults of European ancestry. The high heritability and the constantly increased incidence, which has doubled over the last 20 years, strongly suggest that both genetic and environmental factors are likely to shape the TGCT susceptibility. While genome-wide association studies have identified loci associated with TGCT susceptibility, the role played by environmental molecular vectors in TGCT susceptibility remains unclear.

Introduction: Immunization is the most effective strategy for the prevention of invasive meningococcal disease caused by Neisseria meningitidis serogroup B (MenB); however, parents need to weigh the risk-benefit and financial impact of immunizing their children against MenB in the absence of a national immunization program (NIP). This study aimed to explore societal preferences (of parents and pediatricians) regarding the attributes of a MenB vaccine in Spain.

Methods: A discrete choice experiment (DCE) based on cross-sectional surveys was carried out to determine preferences.

FERTILITY PRESERVATION BEFORE OVARIAN MALIGNANCY TREATMENT While most ovarian epithelial malignancies affect postmenopausal women, 12% occur in reproductive age patients. In addition, borderline ovarian tumors and rare non epithelial ovarian tumors are diagnosed in young patients as well. The prognosis of early-stage epithelial tumors, non-epithelial and frontier tumors is good.

Article Synopsis
  • The study aimed to compare the cost-effectiveness of secukinumab and adalimumab as the first biologic treatment for psoriatic arthritis (PsA) within the Spanish National Health System over a two-year period.
  • Using a cost-consequence analysis, data from 10 hypothetical patients revealed that secukinumab was 18-33% cheaper for achieving different levels of clinical response (ACR20/50/70) compared to adalimumab.
  • The findings suggest that secukinumab may be a more efficient option for treating PsA as a first-line biologic, requiring significant discounts on adalimumab to match its efficacy.

Article Synopsis
  • Klinefelter syndrome (KS) is a leading cause of infertility in genetic males, often resulting in azoospermia, and fertility preservation techniques like TESE are more successful when done early.
  • A systematic review of six studies involving young men (ages 13-24) showed that most participants agreed to sperm collection; however, azoospermia was present in all homogenous KS cases, with some success in mosaic KS.
  • It’s recommended that fertility preservation be proposed for young men with KS, but the ideal age for initiating sperm collection should be tailored to individual circumstances and maturity levels.

This essay analyzes the presence of the modern concept of melancholy in the Intercenales (1424-1439) by the humanist Leon Battista Alberti. The Intercenales is a collection of satirical, allegorical and moralizing writings composed with the purpose of entertaining an audience of close friends. In spite of the fact that the term "melancholia" does not appear in the text, this paper argues that Alberti's character of "the philosopher" is melancholic, since he is "ill in the soul" (suffering from morbus animi), and his illness is evidenced by a series of physical and psychological symptoms associated with melancholy in the classical and medieval medical traditions.
