Transformative changes are needed to support socio-bioeconomies for people and ecosystems in the Amazon.

Current social-technical and political conditions threaten the integrity of the Amazon biome. Overcoming these lock-ins requires structural transformations away from conventional economies towards 'socio-bioeconomies' (SBEs). SBEs are economies based on the sustainable use and restoration of Amazonian ecosystems, as well as Indigenous and rural livelihood systems in the region.

Based on a survey of stakeholders from 34 municipalitiesin Puerto Rico 1 year following HurricaneMaria, this study reports on progress and prioritiesfor longer-term recovery. Following a description ofthe storm and its impacts and an overview of recoveryprinciples, initial insights expressed after the stormare studied. Over 56 percent of respondents rankedthe need for equity considerations as the top recoverypriority, while 23 percent ranked building back faster,and 11 percent supported building back stronger as thetop priority.

An improved preparation of mesoionic heterocycles 1,3-diazolium-4-thiolates by [3 + 2] cycloadditions of münchnones with aryl isothiocyanates is reported. The process takes place with high or complete regioselectivity, and fast and clean transformations are observed under microwave heating in DMF. DFT calculations support that this cycloaddition proceeds preferably through a stepwise mechanism.

Ablation of atrioventricular nodal reentrant tachycardia using remote magnetic guidance (stereotaxis) requires lower temperature and power settings because of improved local contact.

Introduction And Objectives: The Stereotaxis(R) remote magnetic navigation system provides a new approach to ablation that could increase catheter stability. The aim was to determine whether improved tissue contact necessitates a change in traditional radiofrequency ablation parameters.

Methods: The study compared ablation of atrioventricular nodal reentrant tachycardia (AVNRT) using remote navigation (4-mm catheter) in 19 patients with conventional ablation in 18 patients (4-mm catheter, temperature 60-65 degrees C, power 50 W).

Relationship between intraventricular cardiac asynchrony and degree of systolic dysfunction.

Objectives: Cardiac asynchrony is an area of study becoming more relevant in the evaluation and management of heart failure. Our aim was to determine the prevalence of cardiac asynchrony by Doppler echocardiography and to evaluate its relationship with the degree of left ventricular (LV) systolic dysfunction.

Background: The adventitial inversion technique obliterates the false lumen and converts a dissected aorta into a conduit with tough adventitia on the inside and outside. Dacron grafts can be anastomosed to the aorta with fine sutures, which hold without tears.

Right coronary arteries that arise from the left sinus or ectopically from the right sinus with a slit ostium can cause myocardial ischemia, myocardial infarction, or sudden death. The surgical repair of these anomalies has evolved toward a simpler operation that widens the orifice of origin and reduces the compression of the intramural segment. We have repaired these anomalies without mortality in 8 patients.

Subacute sensory neuropathy: a remote effect of carcinoma.

Seven patients with subacute sensory neuropathy are described and the findings in 29 previously reported patients are reviewed. The presence of this characteristic neurological disorder strongly suggests an accompanying neoplasm. Five of 7 patients had cancer, and 4 died.

Transitory cerebral microvascular blockade after cardiopulmonary bypass.

Dogs were submitted to cardiopulmonary bypass (CPB) carried out under conditions calculated to generate large numbers of microbubbles and microemboli. On the day following the procedure the dogs showed evidence of neurological damage including impaired consciousness and ataxia. These abnormalities largely cleared within a week.

Fungal aneurysms of intracranial vessels.

Intracranial fungal aneurysms arise from major cerebral arteries. Fungi directly invade vessel walls from the luminal surface (fungal emboli) or from the adventitia (fungal meningitis). The vasa vasorum are free of fungi.

A demyelinating disorder associated with cerebrovascular amyloid angiopathy.

Five patients with a demyelinating disorder and associated amyloid angiopathy are presented. The disease affected middle-aged individuals, pursued a fluctuating course, and ended in progressive, fatal deterioration of the central nervous system. Neurologic findings indicated multiple lesions within the neuraxis; profound dementia was prominent in all cases.
