Historically, blind intrauterine procedures such as dilation and curettage (D&C) and blind endometrial biopsies have been the primary approach for diagnosing and managing intrauterine pathologies. However, these techniques lack direct visualization, leading to diagnostic limitations, incomplete treatment, and increased complication rates. Despite substantial advances in hysteroscopic technology, including high-definition imaging and minimally invasive instruments, blind procedures remain widely used.

Diabetic kidney disease (DKD) is the leading cause of end stage kidney failure worldwide, of which cellular insulin resistance is a major driver. Here, we study key human kidney cell types implicated in DKD (podocytes, glomerular endothelial, mesangial and proximal tubular cells) in insulin sensitive and resistant conditions, and perform simultaneous transcriptomics and proteomics for integrated analysis. Our data is further compared with bulk- and single-cell transcriptomic kidney biopsy data from early- and advanced-stage DKD patient cohorts.

Lichen sclerosus is a chronic inflammatory condition of unknown etiology that affects the genital and extragenital skin, which can lead to sexual dysfunction and has been associated with vulvar cancer. The vaginal microbiome has a critical role in gynecologic health, but little is known about the microbiome in lichen sclerosus. This study investigated the vaginal and vulvar microbiomes of 27 post-menopausal women with lichen sclerosus.

  • Surgical interventions on the pulmonary valve for adults are rising due to longer lifespans in patients with congenital heart conditions.
  • Reoperations that require multiple chest surgeries have higher risks of complications and mortality.
  • A new minimally invasive technique utilizing a left anterior minithoracotomy shows promising results, with no complications or deaths in a small study of five patients.

  • * Analysis of medical records from a large database compared GBD rates between trans individuals on hormone therapy and those who were not, with findings showing that trans women on estrogen therapy had a significantly higher risk for GBD compared to those not on therapy.
  • * The study concluded that estrogen GAHT is associated with increased GBD risk in transgender women, while transgender men showed no significant change in GBD risk when on testosterone therapy, making this research a first in addressing GBD prevalence in the transgender community.

Gender-affirming surgery (GAS) is a highly personalized decision for transgender and gender diverse (TGD) individuals. However, the proportion of TGD individuals who desire GAS is unknown. A questionnaire was created after identifying themes surrounding experiences with gender-affirming medical care by community focus groups.

Article Synopsis
  • The study aimed to explore patient and healthcare professional (HCP) interactions during hospital admissions by applying temporal network analytics to electronic patient records (EPR) data.
  • By analyzing structured EPR data from renal transplantation admissions between May 2019 and June 2023, researchers constructed individual interaction networks for each admission day, observing how HCPs connected with patients based on their location (ICU or ward).
  • The results indicated that network analysis can reveal intricate patterns and dynamics of HCP interactions, offering valuable insights that traditional analysis methods may miss, which could enhance understanding and operation of hospital patient care.

Background: Chronic pain, affecting approximately 20% of the global population, is the leading cause of disability worldwide. Transgender individuals are disproportionately exposed to chronic pain risk factors compared with the cisgender population. This study compares the incidence of chronic pain between transgender and cisgender individuals and examines the impact of gender affirming hormone therapy, anxiety, and depression on chronic pain.

Several publications have indicated potential benefit from collaboration with industry regarding wider use of anonymised routine NHS healthcare data. However, there is limited guidance regarding exactly how such collaborations between NHS hospitals and industry partners should best be carried out, and specific issues that need to be addressed at an individual project or collaboration level to achieve desired benefit. Specifically, routine health data are complex, not collected in a format optimised for secondary use, and often require interpretation based on clinical understanding of the medical conditions or patients.

Background: Pregnant patients were a significant population to consider during the pandemic, given the impact of SARS-CoV-2 infection on obstetric outcomes. While COVID testing was a central pillar of infection control, it became apparent that a subset of the population declined to test. At the same time, data emerged about pregnant persons also declining testing.

Objective: To synthesize current literature on penile prosthesis implantation post-gender affirming phalloplasty to correlate implant types and complication rates.

Methods: A meta-analysis of penile prosthesis in transmasculine patients was initiated in December 2022. Inclusion criteria encompassed randomized clinical trials, cohort studies, and cross-sectional studies reporting on penile prosthesis complications post-gender-affirming phalloplasty.

Article Synopsis
  • The study explores the use of machine learning (ML) on electronic healthcare record (EHR) data from a pediatric hospital to identify clusters of diseases categorized by patient age, emphasizing the current limitations in data-driven decision-making in hospitals.
  • Using observational data from over 61,000 patients, K-means clustering was applied, resulting in four distinct age clusters for diseases that align with known patterns of illness presentation and progression.
  • The findings highlight the potential of unsupervised ML in enhancing clinical decisions, while also noting that biases and uncertainties in data preprocessing can significantly affect results, necessitating careful communication of such uncertainties.

Objective: To identify clinics in Ohio, Michigan, and Pennsylvania that advertise menopause treatment on their website and evaluate whether clinics not affiliated with a Menopause Society Certified Practitioner (MSCP) are more likely to offer guideline-nonconcordant treatment compared to clinics affiliated with an MSCP.

Methods: We performed an Internet search to identify clinics advertising on their website menopause treatment in Ohio, Michigan, and Pennsylvania. We checked clinic personnel against The Menopause Society directory of practitioners to determine if the clinic was affiliated with an MSCP.

Insulin signaling to the glomerular podocyte via the insulin receptor (IR) is critical for kidney function. In this study we show that near-complete knockout of the closely related insulin-like growth factor 1 receptor (IGF1R) in podocytes is detrimental, resulting in albuminuria and podocyte cell death . In contrast, partial podocyte IGF1R knockdown confers protection against doxorubicin-induced podocyte injury.

Unlabelled: Damage to the articular cartilage resulting in an acute tear can lead to functional changes within the joint and increase the risk of osteoarthritis developing. There is limited understanding of the association between occupational risk factors and sustaining an acute articular cartilage tear in the military and other physically demanding occupations. Therefore, the aim of this systematic review was to identify and evaluate original research reporting on occupational risk factors associated with sustaining acute articular cartilage tears.
