Generalized eczematous reaction to budesonide in a nasal spray with cross-reactivity to triamcinolone.

A 78-year-old woman suffered a generalized eczematous hypersensitivity reaction following the use of an intranasal budesonide inhaler. Patch testing demonstrated positive reactions to both budesonide and triamcinolone. Her eczema responded to emollients, betamethasone dipropionate ointment and cessation of her intranasal budesonide inhaler.

Background: Research on intellectual and cognitive functioning of children of alcoholics has been marked by inconsistency, with some studies unable to document deficits. This discrepancy may reflect the substantial heterogeneity found in the alcoholic population and among families of alcoholics. The current study sought to examine the effects of familial alcoholism subtypes on intellectual, cognitive, and academic performance in early school-aged sons of alcoholics.

Circumferential fingernail.

We describe a Brazilian girl with a congenital circumferential nail on her left ring finger associated with other bony and soft tissue abnormalities of the affected limb. The tubular nail plate resembling a punch biopsy has been described as a circumferential nail, an extremely rare congenital malformation that can be associated with other congenital anomalies. In our review of the literature, there have only been two previous reports of this unusual condition affecting the fingernails.

Cutaneous inflammatory cell infiltrate in chronic idiopathic urticaria: comparison of patients with and without anti-FcepsilonRI or anti-IgE autoantibodies.

Background: Previous studies defining the histopathologic features of patients with chronic idiopathic urticaria (CIU) were performed on wheals of uncertain duration and before the identification of functional autoantibodies against FcepsilonRI and/or IgE, now known to be present in approximately 30% of patients with CIU.

Objective: We sought to determine the timing of the inflammatory infiltrate in the wheals of patients with CIU and to detect differences between patients with and without autoantibodies.

Methods: Immunohistochemistry was used to identify neutrophils (neutrophil elastase), T lymphocytes (CD3), and activated eosinophils (EG2) in biopsy specimens from uninvolved skin and wheals present for less than 4 hours and greater than 12 hours in 22 patients with CIU, as well as in biopsy specimens from the skin of 12 healthy control subjects.

Latent Epstein-Barr virus (EBV) is maintained as a nucleosome-covered episome that can be transcriptionally activated by overexpression of the viral immediate-early protein, Zta. We show here that reactivation of latent EBV by Zta can be significantly enhanced by coexpression of the cellular coactivators CREB binding protein (CBP) and p300. A stable complex containing both Zta and CBP could be isolated from lytically stimulated, but not latently infected RAJI nuclear extracts.

Revision of a total hip arthroplasty with a Harris-Galante porous-coated acetabular component inserted without cement. A follow-up note on the results at five to twelve years.

Fifty-seven revision total hip arthroplasties in fifty-six patients were performed with a Harris-Galante porous-coated acetabular component by one surgeon, and the patients were followed prospectively for a mean of seven years (range, five to twelve years). A trochanteric osteotomy was performed in forty hips, and a posterior approach with an extended anterior capsulectomy was used in the other seventeen. The acetabular defect was classified as segmental in seven hips, cavitary in twenty-three, and combined in twenty-one; six hips had no notable defect.

Results of isolated acetabular revisions: the fate of the unrevised femoral component.

Controversy exists over whether to remove a well-fixed femoral component at the time of revision of a failed acetabular component, and it has been suggested that the femoral component be removed and recemented. The hypothesis presented in this article is that only the acetabular component should be revised in these situations. Thirty-eight isolated, uncemented acetabular revisions were performed by 1 surgeon and prospectively followed for a mean of 4 years (range, 2-10 years).

Comparison of 2D- and 3DFT multiple overlapping thin-slab acquisition TOF MR angiography in carotid disease.

We intended to compare two-dimensional Fourier transform (2DFT) with three-dimensional Fourier transform (3DFT) multiple overlapping thin-slab acquisition (MOTSA) time-of-flight (TOF) magnetic resonance angiography for percent diameter stenosis, length of stenosis, and apparent occlusion at the carotid bifurcation. For that, 101 symptomatic patients (n = 202 carotid bifurcations) were evaluated with 3DFT MOTSA TOF and 2DFT TOF. Three observers measured the percent diameter stenosis from oblique axial multiplanar reformatted (MPR) images and length of stenosis from maximum intensity projection (MIP) images.

To investigate the role of Chlamydia trachomatis in puerperal endometritis, 72 patients with endometritis following vaginal or cesarean section delivery were studied. Blood, urine and endometrial cultures for aerobic and anaerobic bacteria and for C trachomatis were performed for all patients. C trachomatis was isolated in 25% of patients with endometritis.
