Barriers to Clinical Trial Implementation Among Community Care Centers.

Importance: While an overwhelming majority of patients diagnosed with cancer express willingness to participate in clinical trials, only a fraction will enroll onto a research protocol.

Objective: To identify critical barriers to trial enrollment to translate findings into actionable practice changes that increase cancer clinical trial enrollment.

Design, Setting, And Participants: This survey study included designated site contacts at oncology practices with teams who were highly involved with the Association of Community Cancer Centers (ACCC) Community Oncology Research Institute (ACORI) clinical trials activities, all American Society of Clinical Oncology (ASCO)-ACCC collaboration pilot sites, and/or sites providing care to at least 25% African American and Hispanic residents.

Healthcare providers' experiences of continuing care for older adults with cancer during the COVID-19 pandemic.

Purpose: The COVID-19 pandemic has caused great strain on older adults with cancer and their healthcare providers. This study explored healthcare providers' reported changes in cancer care, clinical barriers to care, patient questions, and the overall experiences of caring for older adults with cancer during the COVID-19 crisis.

Introduction: The COVID-19 pandemic has created unprecedented obstacles leading to delays in treatment for older adults with cancer. Goals: The goal was to describe the location of angioectasias within small bowel on capsule endoscopy and the utility of push enteroscopy versus deep enteroscopy in treatment of overt bleeding from these.

Background: Overt bleeding from small bowel angioectasias is a clinical challenge. Thalidomide and octreotide can be difficult to prescribe and may not be effective.

Parathyroid hormone decreases endothelial osteoprotegerin secretion: role of protein kinase A and C.

Parathyroid hormone (PTH), which is elevated in patients with chronic renal failure, has been shown to participate in the development of vascular calcification. Previous studies have demonstrated that PTH may promote endothelial expressions of proinflammatory parameters. On the basis of these data, we evaluated whether PTH may have an impact on endothelial osteoprotegerin (OPG), a vascular-protective factor which may control vascular calcification.

[Stents in dialysis vascular access--do they promise improved high quality prolonged access use].

Unlabelled: The life expectancy of dialysis patients depends, to a large extent, on blood access which provides uninterrupted and efficient treatment. Dialysis access created by a direct anastomosis between artery and vein usually allows normal dialysis for many years. Blood access by a bridge graft between artery and vein functions for a much shorter time and occludes chiefly because of endothelial hyperplasia at the graft vein anastomosis.

Increased detection of early vascular abnormalities after renal biopsies by color Doppler sonography.

Objective: The purpose of this study was to assess the frequency of abnormal vascular findings after renal biopsies as detected by color and pulsed Doppler sonography.

Methods: With both color and pulsed Doppler sonography, we examined 77 patients who underwent a renal biopsy of a native kidney. The examination was carried out before and immediately after the biopsy.

Despeckling of medical ultrasound images using data and rate adaptive lossy compression.

A novel technique for despeckling the medical ultrasound images using lossy compression is presented. The logarithm of the input image is first transformed to the multiscale wavelet domain. It is then shown that the subband coefficients of the log-transformed ultrasound image can be successfully modeled using the generalized Laplacian distribution.

The clinical spectrum of acute renal infarction.

Background: Acute renal infarction is an oft-missed diagnosis. As a result, its true incidence, although presumed to be low, is actually unknown. Surprisingly, the medical literature on the subject, other than anecdotal case reports, is scarce.

Should aspirin therapy be withheld before insertion and/or removal of a permanent peritoneal dialysis catheter?

The necessity of withdrawal of aspirin [acetylsalicylic acid (ASA)] for fear of perioperative or postoperative bleeding in patients about to undergo surgery is as yet controversial. In this study we prospectively evaluated the effect of ASA on postoperative bleeding in end-stage renal failure patients who underwent insertion, removal, and/or replacement of a peritoneal dialysis (PD) catheter at our institution from November 1999 to March 2001. During the study period 52 of the above procedures were consecutively performed in 46 patients.

Portable pupillography of the swinging flashlight test to detect afferent pupillary defects.

Objective: To investigate the ability of a portable, personal computer-driven, pupillometer to record the pupillary response curve during the swinging flashlight test. Also, to determine whether these response curves can be used to identify and quantify relative asymmetry in the pupillary light reflex between eyes in healthy volunteers with simulated afferent pupil defects (APDs) and patients with optic neuropathies.

Design: Comparative, observational case series and instrument validation.

The in vivo assessment of tibial motion in the transverse plane in anterior cruciate ligament-reconstructed knees.

Twenty-one knees with acutely injured anterior cruciate ligaments were reconstructed with patellar tendon autografts. Eight of the knees had concomitant medial ligament injuries that were not addressed surgically. Follow-up evaluation (average, 25 months) included computed tomography measurements to analyze transverse-plane laxity in both translation and rotation.
