Article Synopsis
  • Liver transplantation (LT) is an effective treatment for children with severe liver diseases, and King Chulalongkorn Memorial Hospital in Thailand has performed many pediatric cases.
  • The study analyzed 94 pediatric LT patients from 2004 to 2019, noting that the majority received grafts from living-related donors, revealing shorter wait times than those from deceased donors.
  • The common diagnosis among patients was biliary atresia, and infections were the main postoperative complication; the data also highlighted ABO incompatibility in 9% of transplants without prior desensitization.

A high prevalence of hepatitis B (HepB) antibody loss after liver transplantation (LT) and de novo HepB infection (DNH) was documented, hence revaccination to prevent DNH is crucial. This study aimed to compare the safety and immunogenicity of two HepB vaccine regimens in liver-transplanted children. Liver-transplanted children who were previously immunised but showed HepB surface antibodies (anti-HBs) ≤ 100 mIU/mL were randomised to receive a standard three-dose (SD) and double three-dose (DD) vaccine intramuscularly in months 0-1-6.

Article Synopsis
  • * Methods: Researchers reviewed medical records of children with chronic liver diseases who received hepatitis B immunization, focusing on their anti-HBs levels before and after transplantation over a period from May 2001 to June 2017.
  • * Results: Out of 50 children, the study found that the rate of non-immunity significantly increased over time after liver transplantation, with nearly 82% showing low anti-HBs levels after three years. A specific case of a boy who developed hepatitis B infection despite high pre-transplant vaccine titers was also detailed.
