Neonatal screening has excellent coverage in France. Data from the foreign literature raise questions about the informed consent to this screening. The Neonatal Screening and Informed Consent (DENICE) study was designed to assess whether information on neonatal screening provided for families in Brittany allows for informed consent.

Patent Foramen Ovale Closure or Anticoagulation vs. Antiplatelets after Stroke.

Background: Trials of patent foramen ovale (PFO) closure to prevent recurrent stroke have been inconclusive. We investigated whether patients with cryptogenic stroke and echocardiographic features representing risk of stroke would benefit from PFO closure or anticoagulation, as compared with antiplatelet therapy.

Methods: In a multicenter, randomized, open-label trial, we assigned, in a 1:1:1 ratio, patients 16 to 60 years of age who had had a recent stroke attributed to PFO, with an associated atrial septal aneurysm or large interatrial shunt, to transcatheter PFO closure plus long-term antiplatelet therapy (PFO closure group), antiplatelet therapy alone (antiplatelet-only group), or oral anticoagulation (anticoagulation group) (randomization group 1).

[Withdrawal of artificial nutrition and hydration in severe stroke: medical, legal and ethical considerations].

In the majority of cases, severe stroke is accompanied by difficulty in swallowing and an altered state of consciousness requiring artificial nutrition and hydration. Because of their artificial nature, nutrition and hydration are considered by law as treatment rather basic care. Withdrawal of these treatments is dictated by the refusal of unreasonable obstinacy enshrined in law and is justified by the risk of severe disability and very poor quality of life.

Internal consistency and validity of the Spanish version of the quality of life questionnaire specific for obstructive sleep apnea: sleep apnea quality of life index.

Introduction: The objective of the present study was to validate the Spanish version of the SAQLI, which is a health-related quality of life (HRQL) questionnaire specific for sleep apnea-hypopnea syndrome (SAHS), and to assess its sensitivity to change.

Material And Methods: A multicenter study performed in a group of patients with SAHS (apnea-hypopnea index [AHI] ≥5) who had been referred to the centers' Sleep Units. All patients completed the following questionnaires: SF-36, FOSQ, SAQLI and Epworth scale.

Hereditary spastic paraplegias (HSP) constitute a heterogeneous group of neurodegenerative disorders characterized at least by slowly progressive spasticity of the lower limbs. Mutations in REEP1 were recently associated with a pure dominant HSP, SPG31. We sequenced all exons of REEP1 and searched for rearrangements by multiplex ligation-dependent probe amplification (MLPA) in a large panel of 175 unrelated HSP index patients from kindreds with dominant inheritance (AD-HSP), with either pure (n = 102) or complicated (n = 73) forms of the disease, after exclusion of other known HSP genes.

Naturally occurring variations in defensive burying behavior are associated with differences in vasopressin, oxytocin, and androgen receptors in the male rat.

Largely ignored in tests of defensive burying is the capacity for individual animals to display marked variations in active coping behaviors. To expose the neurobiological correlates of this behavioral differentiation rats were exposed to a mousetrap that was remotely triggered upon approach to remove the quality of pain. Relative to animals showing no significant levels of defensive burying activity, rats showing sustained elevations in defensive burying displayed higher levels of arginine vasopressin (AVP) mRNA and increased numbers of androgen receptor positive cells in the medial amygdala and posterior bed nuclei of the stria terminalis, brain regions that integrate emotional appraisal and sensory information.

Introduction: Atherothrombosis is a systemic, diffuse disease associated with a high risk of cardiovascular morbidity and mortality. It is the main cause of death in Western populations, a major public health concern and its prevalence will further increase in the future.

Objectives: To evaluate the rate of major vascular events at 1 year in French patients with confirmed atherothrombotic disease, recruited in the REACH international registry.

Laser relaxation-oscillation frequency imaging.

We describe a new imaging technique based on modification of laser relaxation frequency induced by coherent optical feedback from an external target. A direct comparison (both theoretical and experimental) is made with laser feedback interferometry techniques, in which there is a modification of the laser's steady state. We show that, for a laser with a cavity damping rate gamma(c) higher than the population damping rate, gamma(1) , the modification of the laser relaxation frequency can be several orders of magnitude more sensitive than the perturbation of the laser's output power.

Prognostic factors in the recovery of the ability to walk after stroke.

We have studied the recovery of walking ability on being discharged from a department of physical medicine and rehabilitation in patients with hemiplegia after stroke, and the factors influencing this recovery. This prospective study was based on 93 patients. The patients, who were considered to be ambulatory, were able to move 10 metres on their own or with supervision when they were discharged.

Effects of expectation on amygdala-kindled convulsions and interictal behaviour in rats: confirmation of a Pavlovian mechanism.

We found evidence that the effects of exposure to the stimulation environment on the convulsions and interictal behaviour of basolateral amygdala (BA)-kindled rats are the result of Pavlovian conditioning. In Experiment 1, the rats first received 45 BA stimuli in one environment (CS+) and 45 sham stimuli in another environment (CS-). During this first phase of the experiment, the CS+ and CS- began to exert differential effects on interictal behaviour and convulsions.

Antidepressant effects of exercise: evidence for an adult-neurogenesis hypothesis?

It has been hypothesized that a decrease in the synthesis of new neurons in the adult hippocampus might be linked to major depressive disorder (MDD). This hypothesis arose after it was discovered that antidepressant medications increased the synthesis of new neurons in the brain, and it was noted that the therapeutic effects of antidepressants occurred over a time span that approximates the time taken for the new neurons to become functional. Like antidepressants, exercise also increases the synthesis of new neurons in the adult brain: a 2-3-fold increase in hippocampal neurogenesis has been observed in rats with regular access to a running wheel when they are compared with control animals.

Conditioned effects of kindling three different sites in the hippocampal complex of the rat.

Rats received kindling stimulations to the perirhinal cortex (PRh), ventral hippocampus (VH), or dorsal hippocampus (DH) in 1 environment and an equivalent number of sham stimulations in a 2nd environment. The PRh-kindled rats displayed rapid kindling and a swift emergence of conditioned interictal defensiveness. In contrast, the VH- and DH-kindled rats displayed much slower kindling and slow or no conditioning, respectively.

Serotonin receptor binding and mRNA expression in the hippocampus of fearful amygdala-kindled rats.

Amygdala kindling in rats increases fear behavior. The neural correlates of this fear are not well understood. In this experiment, we investigated the relation between serotonin receptor binding and mRNA expression and fearful behavior in amygdala-kindled rats.

Long-term kindling of the basolateral amygdala impairs copulatory behavior in male rats.

This experiment examined the consequences of long-term kindling of the basolateral amygdala on male sexual behavior and the frequency of both spontaneous wet dog shakes (WDS) and those induced by the 5-HT2A receptor agonist DOI. Results demonstrated that following 60 stimulations of the left basolateral amygdala over a 4-week period, male Long-Evans rats exhibited decrements in every aspect of sexual behavior. Specifically, latencies to mount, intromit and ejaculate were all prolonged following long-term kindling, and ejaculation frequencies were significantly reduced.

Anticipating the attack: temporal conditioning during amygdala kindling in rats.

The present study showed that amygdala-kindled rats use short-interval timing superimposed on phase or ordinal timing to predict when a convulsion will occur. In 2 experiments, rats received 1 stimulation and 1 sham stimulation each day, always at the same times (conditioned stimulus [CS]+ and CS- times, respectively) and 150 s after rats had been placed in the testing chamber (the preadministration interval). As kindling progressed, the rats displayed more defensive behavior at the CS+ time than at the CS- time.

[Systemic sarcoidosis revealed by acute ataxic sensory polyradiculoneuropathy].

We present the case of a 35-year-old man who developed bilateral uveitis and acute ataxic sensorial polyradiculoneuropathy with sphincter dysfunction. The patient had multiple mediastinal adenopathies. Pathology examination led to the diagnosis of sarcoidosis.
