Article Synopsis
  • Limited data exist on the outcomes of inflammatory bowel disease (IBD) in patients who undergo solid organ transplantation (SOT), and this study examines both pre-existing IBD and newly developed IBD post-transplant.
  • The study included 177 patients, of which 106 had pre-existing IBD, and found that 32% of these patients experienced disease progression after a median of 2.2 years, while 55% of those with de novo IBD progressed within 1.9 years.
  • Key risk factors for progression in pre-existing IBD included having active disease at the time of SOT and extraintestinal manifestations, indicating a significant impact on patient outcomes post-transplant.*

Ustekinumab has shown efficacy in Crohn's Disease (CD) patients. To identify patient profiles of those who benefit the most from this treatment would help to position this drug in the therapeutic paradigm of CD and generate hypotheses for future trials. The objective of this analysis was to determine whether baseline patient characteristics are predictive of remission and the drug durability of ustekinumab, and whether its positioning with respect to prior use of biologics has a significant effect after correcting for disease severity and phenotype at baseline using interpretable machine learning.

Intestinal failure (IF) is the inability of the gut to absorb necessary water, macronutrients, micronutrients, and electrolytes sufficient to sustain life and requiring intravenous supplementation or replacement. IF Types 1 and 2 are the initial phase of this condition and usually last for weeks to a few months. Type 3 IF (also known as chronic IF [CIF]) is a chronic and stable condition, usually irreversible, whose main treatment is home parenteral nutrition.

Background: Large real-world-evidence studies are required to confirm the durability of response, effectiveness, and safety of ustekinumab in Crohn's disease (CD) patients in real-world clinical practice.

Methods: A retrospective, multicentre study was conducted in Spain in patients with active CD who had received ≥1 intravenous dose of ustekinumab for ≥6 months. Primary outcome was ustekinumab retention rate; secondary outcomes were to identify predictive factors for drug retention, short-term remission (week 16), loss of response and predictive factors for short-term efficacy and loss of response, and ustekinumab safety.

Non-alcoholic fatty liver disease (NAFLD) is currently considered the most common cause of liver disease. Its prevalence is increasing in parallel with the obesity and type 2 diabetes mellitus (DM2) epidemics in developed countries. Several recent studies have suggested that NAFLD may be the hepatic manifestation of a systemic inflammatory metabolic disease that also affects other organs, such as intestine, lungs, skin and vascular endothelium.

Background: The effectiveness of the switch to another anti-tumor necrosis factor (anti-TNF) agent is not known. The aim of this study was to analyze the effectiveness and safety of treatment with a second and third anti-TNF drug after intolerance to or failure of a previous anti-TNF agent in inflammatory bowel disease (IBD) patients.

Methods: We included patients diagnosed with IBD from the ENEIDA registry who received another anti-TNF after intolerance to or failure of a prior anti-TNF agent.

Article Synopsis
  • Primary sclerosing cholangitis (PSC) is often linked with inflammatory bowel disease (IBD) and is associated with a higher risk of cancers, especially colorectal cancer (CRC) and cholangiocarcinoma (CCA).
  • A study of 277 PSC-IBD patients revealed a PSC incidence rate of 61 cases per 100,000 IBD patient-years, with the majority treated using ursodeoxycholic acid and some requiring liver transplants.
  • The findings indicate that PSC symptoms at diagnosis increase CRC risk, with CCA found primarily in patients with intra- and extrahepatic PSC, leading to poorer survival compared to those without CCA.

To evaluate outcomes of early dose optimization of golimumab in ulcerative colitis (UC) patients with inadequate response to golimumab induction treatment. This observational, multicenter, cohort study included patients with moderate-to-severe active UC and with inadequate response to subcutaneous golimumab induction doses, in whom weight-based golimumab maintenance dose (European labeling) of 50 mg every 4 weeks (q4wk) was optimized before week 14 to 100 mg q4wk. At week 14, we assessed clinical response and remission using the partial Mayo score.

The administration of propofol by endoscopists is a source of permanent friction with the Societies of Anesthesiology, which is based more on a clear conflict of economic interest on the part of the anesthesiologists than supported by scientific evidence. Maestro Antolín et al. (1) presented a series of more than 33,000 sedations performed with propofol by endoscopists, observing a frequency of cardiorespiratory adverse events of 0.

Article Synopsis
  • This study evaluated golimumab effectiveness in treating ulcerative colitis (UC) patients who were either new to anti-TNF therapies or had already undergone treatment.
  • Approximately 65% of the 142 patients experienced a short-term clinical response, with better outcomes seen in those using golimumab as their first anti-TNF.
  • Despite lower response rates in patients with prior anti-TNF failures, golimumab remained effective overall, and dosage increases helped many regain response.

Inflammatory bowel diseases are inflammatory, chronic and progressive diseases of the intestinal tract for which no curative treatment is available. Research in other fields with stem cells of different sources and with immunoregulatory cells (regulatory T-lymphocytes and dendritic T-cells) opens up new expectations for their use in these diseases. The goal for stem cell-based therapy is to provide a permanent cure.

Background And Aims: While it is commonly accepted that Inflammatory bowel disease (IBD) Comprehensive Care Units (ICCUs) facilitate the delivery of quality care to Crohn's disease and ulcerative colitis patients, it remains unclear how an ICCU should be defined or evaluated. The aim of the present study was to develop a comprehensive set of Quality Indicators (QIs) of structure, process, and outcomes for defining and evaluating an ICCU.

Methods: A Delphi consensus-based approach with a standardized three-step process was used to identify a core set of QIs.
