Article Synopsis
  • The study aimed to analyze lung cancer patients who had surgery for tumors invading the chest wall, comparing outcomes based on whether they received induction chemotherapy, induction radiochemotherapy, or no induction therapy.
  • Out of 688 patients studied, those who received induction radiochemotherapy had the best long-term survival rates, while postoperative mortality and incomplete resection rates varied among the groups.
  • The findings suggest that induction radiochemotherapy could improve survival outcomes, indicating a need for further research through a prospective randomized trial.

Introduction: Stage III NSCLC comprises a heterogeneous population. Different treatment strategies are available, including surgery, radiotherapy, and chemotherapy. The PACIFIC trial results represented a significant change and improvement in the therapeutic strategy for these patients.

Article Synopsis
  • The study investigates the management of infections related to synthetic grafts used in the superior vena cava between 2001 and 2018, based on data from the French EPITHOR database.
  • Out of 111 patients studied, 12 developed graft infections primarily due to contamination, with management including either conservative treatment or a surgical approach depending on the severity of the infection.
  • The results indicate a 25% operative mortality rate and suggest that while graft removal isn't always required, it should be considered for severe or recurrent infections.

Sarcomas are rare tumours that represent less than 1% of all malignant tumours in adults. Liposarcomas are among the most common malignant mesenchymal tumours. They are preferentially located in the limbs and the retroperitoneum.

Introduction: Surgical management of Diaphragmatic and thoracic endometriosis (DTE) is still controversial, a thoracic or an abdominal approach can be proposed.

Methods: We conducted a multicentric retrospective study in 8 thoracic, gynecology or digestive surgery units in 5 French university hospitals. The main objective was to review the current management of DTE.

Objectives: Home-based rehabilitation programmes (H-RPs) could facilitate the implementation of pulmonary rehabilitation prior to resection for non-small cell lung cancer (NSCLC), but their feasibility has not been evaluated. The aim of this study was to identify determinants of non-completion of an H-RP and the factors associated with medical events occurring 30 days after hospital discharge.

Design: A prospective observational study.

Article Synopsis
  • A patient with multiple injuries sustained a flail chest from a climbing fall, resembling a condition called pectus excavatum.
  • The treatment involved the use of a vacuum bell, which helped increase the patient's deep breath, peak flow, and pectus index.
  • The approach combined daytime vacuum bell use with continuous positive airway pressure at night for six weeks, successfully stabilizing the flail chest.

The esophagus traverses three body compartments (neck, thorax, and abdomen) and is surrounded at each level by vital organs. Injuries to the esophagus may be classified as foreign body ingestion, caustic ingestion, esophageal perforation, and esophageal trauma. These lesions can be life-threatening either by digestive contamination of surrounding structures in case of esophageal wall breach or concomitant damage of surrounding organs.

Background: Stage IV non-small cell lung cancer (NSCLC) is considered incurable; however, some patients with only few metastases may benefit from treatment with a curative intent. We aimed to identify the prognostic factors for stage IV NSCLC with synchronous solitary M1.

Methods: A database constructed from our weekly multidisciplinary thoracic oncology meetings was retrospectively screened from 1993 to 2012.

Purpose: Surgical esophagectomy is the gold standard treatment of early-stage esophageal cancer. The procedure is complicated with significant morbidity; the most severe complication being the anastomotic leakage. Anastomotic fistulas are reported in 5-25% of cases and are mainly due to gastric transplant ischemia.

Objective: The objective of this study was to determine the efficacy of alginate staple-line reinforcement of fissure openings as compared with stapling alone, with or without tissue sealant or glue, in reducing the incidence and duration of air leakage after pulmonary lobectomy for malignancy.

Summary Background Data: No randomized trial evaluating alginate staple-line reinforcement has been performed to date.

Methods: The Staple-line Reinforcement for Prevention of Pulmonary Air Leakage study was a multicenter randomized trial, with blinded evaluation of endpoints.

We report the first case of endovascular covered stent implantation for the treatment of a large pulmonary artery pseudoaneurysm (PAPA) following a right thoracic gunshot wound. After resuscitation and hemodynamic stabilization, a CT angiography was performed to analyze the neck size of the PAPA and its position relative to the branches of the parent artery. Covered stent implantation with additional coil embolization was successfully performed.

We present a case of a giant (13-cm length) purely polypoid esophageal leiomyoma without any intramural development. This form of leiomyoma is rare and develops due to proliferation originating from the muscularis mucosae, although the intramural type originates in the muscularis propria. This should not be confused with giant fibrovascular polyps, which are postulated to arise at the pharyngoesophageal junction when a flap of mobile, redundant submucosa prolapses distally and may cause asphyxia when protruding into the mouth.

Article Synopsis
  • The French organ transplantation authorities established the 'high-emergency waiting list' (HEWL) in 2007 to address the high mortality rate among patients on the regular waiting list for lung transplantation (LTx).
  • A study analyzing data from 101 HEWL patients showed that 95 received transplants, with a median wait time of 4 days, while 6 patients died before receiving LTx; the patients commonly suffered from conditions like cystic fibrosis and pulmonary fibrosis.
  • Despite the HEWL improving access to LTx for patients in respiratory distress, it also resulted in a higher risk of post-transplant mortality, with survival rates at one and three years being 67.5% and 59%, respectively.

Activation of normally silent tissue-specific genes and the resulting cell "identity crisis" are the unexplored consequences of malignant epigenetic reprogramming. We designed a strategy for investigating this reprogramming, which consisted of identifying a large number of tissue-restricted genes that are epigenetically silenced in normal somatic cells and then detecting their expression in cancer. This approach led to the demonstration that large-scale "off-context" gene activations systematically occur in a variety of cancer types.

A 41-year-old woman had a jeopardizing air leak from an alveolar-pleural and transdiaphragmatic fistula with pulmonary cavitation, secondary to a severe postoperative abdominal sepsis. Her condition dramatically improved by introduction, in the lower bronchus, of a one-way endobronchial valve, leading to immediate cessation of air leakage and removal of extracorporeal membrane oxygenation, and thus avoiding a lower left lobectomy with myoplasty. Furthermore, removal of the valve nine weeks later led to near-complete recovery of the left lower lobe.

The extraction by cervicotomy of substernal goitres may be impossible and sometimes requires the enlargement of the thoracic inlet with at least a sternal-split. We present the extraction of a posterior mediastinal substernal goitre with the application of an innovative vacuum-based suction device, previously used for the control of bleeding from the heart and great vessels in clinical and experimental conditions.

We describe a technique of correction of a severe pouter pigeon breast. Three osteotomies were performed on the sternum, one resecting the curved fused angle of Louis. Multiples chondrotomies allowed to flatten the cartilage part.
