Article Synopsis
  • The study evaluates the effectiveness of ticagrelor compared to clopidogrel in older adults with acute coronary syndrome undergoing percutaneous coronary intervention.
  • It included a cohort of 42,843 Medicare beneficiaries, with 23% classified as frail, and analyzed data on major adverse cardiovascular events and major bleeding following treatment.
  • The results showed a slight reduction in cardiovascular events and an increase in bleeding risk with ticagrelor for non-frail patients, while showing insufficient evidence for its benefits in frail older adults.

Article Synopsis
  • Pleural effusion is a frequent reason for hospital admissions, often requiring thoracentesis for diagnosis, and this study investigated the effectiveness of ultrasound imaging (TUS) in distinguishing between two types of pleural fluid: transudative and exudative.
  • A total of 166 patients were analyzed, with 74% of their pleural effusions classified as exudative by Light's Criteria; TUS identified anechoic effusions in 71% of cases.
  • While some TUS features like septation were highly specific for exudative effusions, the overall imaging characteristics did not reliably differentiate between transudative and exudative fluids, particularly in patients with underlying conditions like heart failure or malignancy

Continuous-flow left ventricular assist devices (CF-LVAD) have been shown to enhance reverse remodeling and myocardial recovery in certain patients allowing for device removal. We sought to analyze the characteristics and describe outcomes of patients who underwent CF-LVAD explantation at a large academic center. We retrospectively identified all patients who underwent CF-LVAD explants due to recovery from 2006 to 2019.

Background: Glucocorticoid treatment remains the cornerstone of therapy for immune checkpoint inhibitor (ICI) myocarditis, but data supporting the use of additional immunotherapy for steroid refractory cases remains limited. We investigate the safety and efficacy of infliximab in patients with ICI myocarditis who are refractory to corticosteroids. Additionally, we highlight the importance of a multi-disciplinary approach in the care for these complex patients.

Purpose: To examine arthroscopic treatment of hip pain in patients with borderline hip dysplasia (lateral center edge angle [LCEA] between 20° and 25°).

Methods: From 2008 to 2013, patients below 18 years of age who underwent arthroscopic hip surgery with an LCEA between 20° and 25° were retrospectively matched 1:1 to a control group without dysplasia (LCEA ≥25°) based on age, gender, femoroplasty, labral treatment, and capsular plication. Indications for surgery included failure to improve with nonoperative treatments and anti-inflammatory medications after 3 months.
