Arthroscopic suprascapular nerve decompression: transarticular and subacromial approach.

Entrapment of the suprascapular nerve (SSN) is an increasingly recognized entity that can occur due to traction or compression-related etiology. Traction injuries of the SSN are unlikely to respond to surgical management and frequently improve with rest and avoidance of overhead activity. Compression injuries, on the other hand, frequently require surgical decompression for pain relief.

Degenerative joint disease of the acromioclavicular joint: a review.

Osteoarthritis of the acromioclavicular (AC) joint is a common condition causing anterior or superior shoulder pain, especially with overhead and cross-body activities. This most commonly occurs in middle-aged individuals because of degeneration to the fibrocartilaginous disk that cushions the articulations. Diagnosis relies on history, physical examination, imaging, and diagnostic local anesthetic injection.

Anteromedial versus transtibial tunnel drilling in anterior cruciate ligament reconstructions: a systematic review.

Purpose: Failure to anatomically reconstruct the femoral footprint can lead to rotational instability and clinical failure. Thus we sought to compare femoral tunnel drilling techniques, specifically anteromedial (AM) and transtibial (TT) methods, with respect to rotational stability.

Methods: In this study we evaluated available scientific support for the ability of both techniques to achieve rotational stability of the knee through a systematic review of the literature for directly comparative biomechanical and clinical studies.

Revision posterior shoulder stabilization.

Background: Revision arthroscopic posterior glenohumeral stabilization requires a thorough understanding of the static and dynamic stabilizers of the glenohumeral joint. The evaluation of these patients is complex but critical given the variety of possible underlying lesions.

Method: We reviewed the literature surrounding recurrent and revision posterior instability biomechanics, etiology, evaluation, treatment, and outcomes.

Surgical management of osteochondritis dissecans of the knee.

Osteochondritis dissecans of the knee primarily affects subchondral bone, with a secondary effect on the overlying articular cartilage. This process can lead to pain, effusions, and loose body formation. While stable juvenile lesions often respond well to nonoperative management, unstable juvenile lesions, as well as symptomatic adult lesions, often require operative intervention.

Return to high-level sport after meniscal allograft transplantation.

Purpose: Our purpose was to determine the results of meniscal allograft transplantation (MAT) in high school and higher-level athletes, specifically with respect to return to their preinjury level of play.

Methods: This is a retrospective case series of prospectively collected data from a single surgeon, evaluating athletes who underwent MAT. Each patient participated in either high school, collegiate, or professional athletics before injury, with a mean preinjury Tegner score of 8.

Graft tensioning during knee ligament reconstruction: principles and practice.

Failure to correctly tension grafts may overconstrain or underconstrain the knee, potentially predisposing the patient to deteriorating clinical and/or radiographic results over time. Knee ligament reconstruction requires a fundamental understanding of native anatomy, ligament biomechanics, and principles of graft tensioning. A successful strategy for graft tensioning takes into account the specific biomechanics of the ligament or ligaments in question, the mechanical properties of the graft selected, the chosen fixation method, the selected tensioning method (ie, manual or mechanical), and the overall goal of the reconstruction (ie, isometry versus anisometry).

Operative treatment of chondral defects in the glenohumeral joint.

Purpose: The objectives of this study were to conduct a systematic review of clinical outcomes after cartilage restorative and reparative procedures in the glenohumeral joint, to identify prognostic factors that predict clinical outcomes, to provide treatment recommendations based on the best available evidence, and to highlight literature gaps that require future research.

Methods: We searched Medline (1948 to week 1 of February 2012) and Embase (1980 to week 5 of 2012) for studies evaluating the results of arthroscopic debridement, microfracture, osteochondral autograft or allograft transplants, and autologous chondrocyte implantation for glenohumeral chondral lesions. Other inclusion criteria included minimum 8 months' follow-up.

Cutaneous manifestation of metallosis in a metal-on-metal total hip arthroplasty after acetabular liner dissociation.

In this case report, we describe a cutaneous manifestation of extensive metallosis in a patient 4 months post-metal-on-metal total hip arthroplasty with a Pinnacle cup with dissociation of the liner from the shell and resultant stripe burnishing of the shell and notch wear of the femoral neck. Dissociation of a metal liner has not been previously reported with this implant. Cutaneous metallosis has only been reported once in the literature.

Rheumatoid synovectomy: does the surgical approach matter?

Background: Surgical synovectomy relieves pain in patients with rheumatoid arthritis (RA). The comparative effect of arthroscopic versus open synovectomy on pain reduction, recurrence of synovitis, radiographic progression, and need for subsequent total joint arthroplasty (TJA) is unclear. Whether synovectomy relieves pain in patients with advanced degenerative joint changes is also controversial.

Acute, avulsion fractures of the medial epicondyle while throwing in youth baseball players: a variant of Little League elbow.

Hypothesis: The young throwing athlete is susceptible to medial elbow injury due to valgus overload. We hypothesized that this injury can occur during the throwing motion with an acute episode of medial elbow pain resulting in an inability to effectively participate in throwing activities. In addition, appropriate treatment of acute, medial epicondyle avulsion fractures in baseball players can result in an asymptomatic elbow with subsequent return to play within a year of injury.

Pediatric osteogenic sarcoma.

Purpose Of Review: Osteogenic sarcoma, also known as osteosarcoma, is a primary malignancy of the bone in which spindle cells produce osteoid. Although rare in the general population, these tumors are the most common primary malignancies of bone and the fifth most common primary malignancy of adolescence. This paper aims to provide a general overview on the presentation, radiographic evaluation, and treatment of osteosarcoma as it presents in the pediatric patient population.
