Initial experience of parenchyma-sparing liver resection with systematic selective hepatic vein reconstruction for colorectal metastases.

Objectives: This study aims to assess the feasibility and short-term and intermediate-term technical success rate of the concept of systematic selective hepatic vein (HV) reconstruction for parenchyma-sparing hepatectomies (PSHs) in patients with colorectal liver metastases (CRLM) in accordance with stage 2a of the IDEAL framework.

Design: The prospective case series of patients deemed eligible and operated on according to the concept.

Setting: All patients were treated by a single surgical team in three hospitals in Ukraine from June 2022 to November 2023.

Phonon Topology and Winding of Spectral Weight in Graphite.

The topology of electronic and phonon band structures of graphene is well studied and known to exhibit a Dirac cone at the K point of the Brillouin zone. Here, we applied inelastic x-ray scattering (IXS) along with ab initio calculations to investigate phonon topology in graphite, the 3D analog of graphene. We identified a pair of modes that form a very weakly gapped linear anticrossing at the K point that can be essentially viewed as a Dirac cone approximant.

[Effects of prostacyclin MM-706 on liver function restoration in dogs after severe hemorrhagic shock].

The new synthetic analogue of prostaglandin I2 (prostacyclin MM-706) influence on some hepatic function indexes was studied up in 7 dogs with an acute hepatic insufficiency caused by severe hemorrhagic shock. Rapid and steady normalization of indexes and the animals survival were promoted by intravenous infusion of MM-706 given in 300 ng/kg dose together with hemocorrector named lactoprotein given in 10 ml/kg dose (7,5 +/- 1,4) hours later. Possible action mechanisms of the MM-706 therapeutic efficacy are considered.

[Contents of prostaglandins F2alpha, I2 (prostacyclin) and thromboxane A2 in the dog liver during development of hemorrhagic shock].

Contents of prostaglandins (PGs) F2 alpha, I2 and thromboxane (TH) A2 in the liver of 6 dogs has been studied in dynamics of hemorrhagic shock development. The results show that acute haemorrhage (31.9 +/- 1.

[The expediency of the clinical use of prostaglandin E2 for treating acute renal failure].

The effect of exogenous prostaglandin (PG) E2 ("PROSTENON") on the renal function and some metabolic parameters have been studied in 10 dogs with acute renal failure (ARF) caused by the crush-syndrome. It has been shown that intravenous PGE2 at a dose of 0.25 mg/kg 1 h after relief of soft tissue compression leads to prompt and stable recovery of all the renal functions under study, normalises metabolism and promotes the survival of animals.

[Use of allograft of pancreatic islet cell cultures in complex treatment of acute purulent process in patients with diabetes mellitus].

The results of treatment of 170 patients with diabetes mellitus and purulent wounds which appeared after opening the abscesses and phlegmons, and as well of 270 patients with gangrene of the lower extremities are presented. Early opening the abscess, improved quality of debridement of purulent wounds, and as well allotransplantation of pancreatic islet cell cultures (45 cases) permitted to stop progressing of the purulent process, place early secondary sutures, and in patients with the sutures placed, to achieve primary healing of the wounds.

[The clinico-morphological characteristics of thyroid cancer in a region of endemic goiter].

The study deals with the results of clinico-morphologic analysis of thyroid cancer in 351 patients who have been operated on in the Carpathian Mountain [correction of Prikarpatsky] region endemic for goiter over the period of 25 years. Certain peculiarities of thyroid cancer were established: relatively high occurrence of the disease, specific ratio of basic histologic subtypes, high frequency of early stage cases and regular concomitance of cancer with other thyroid pathologies. The importance of further studies into the epidemiology of thyroid cancer is emphasized.

[Long-term results of allotransplantation cultures of pancreatic islets cells].

Under study were long-term results of transplantation of cultures of islet cells of the pancreas of human fetuses in 95 patients. A marked antidiabetic effect of transplantation was noted in 89 recipients. A conclusion was made that recipients with diabetes of the middle and first stages are not very perspective for intramuscular transplantation of the islet cell cultures of the pancreas.

[Intraportal allogeneic transplantation of the islets of Langerhans cultures in the complex treatment of surgical patients with diabetes mellitus].

The article presents results of treatment of 21 patients with a surgical pathology and diabetes mellitus who had intraportal allotransplantation of cultures of islet cells of the pancreas and 316 patients with the similar surgical pathology without such transplantation. It was found that the intraportal introduction of islet cell cultures into the liver can result in an early and considerable drop of the injected insulin dose which is very important in the postoperative period. This method allowed to make the period of treatment at the hospital approximately 15 days shorter.

[The content of prostaglandins A and E in the blood and renal tissues in the early phase of experimental acute renal failure caused by the crush syndrome].

Prostaglandin (PG) A and E content in the blood and renal tissues (cortex and medulla) has been studied in 25 dogs in the early phase of acute renal failure (ARF). It has been found that by the 4th hour of compression PG content increases drastically in the blood and renal cortex and decreases in the renal medulla. Compression relief and ARF progress were accompanied by a drop in PG concentration both in the blood and the renal cortex and medulla.

[Disorders of the motor and evacuatory functions of the stomach and duodenum in peptic ulcer patients].

Complex examination of 73 patients with ulcer disease (bulbus duodeni location) revealed regularities of evacuation of a test breakfast from the stomach depending on the phase of the disease. Significant disorders of the motor-emptying gastric and duodenal function were found that are to be considered in the treatment of this category of patients.

[Ultrasonography of the abdominal cavity organs in peptic ulcer patients following gastric resection].

A study of 205 patients in the follow-up period after gastric resection for ulcer disease indicates that ultrasonography is a valuable method of diagnosis of involvement of the abdominal cavity organs but requires perfection of methods directed at improvement of visualization of the scanned organs. For diagnosis of diffuse lesions of the liver it is rational to combine ultra-sonography with radionuclide, endoscopic and roentgenological methods. It was found that patients with gastric resection syndromes, calculosis of the gallbladder was present in 17.

[Cytopathomorphism of the thyroid].

Despite the successes in anti-goitre actions, the problem of thyroid pathology still remains to be of high complexity and concern. Research conducted in the endemic goitre focus in the Pre-Carpathians indicated the presence of pronounced pathomorphosis of thyroid abnormalities. The incidence of autoimmune and malignant conditions is growing, so is that of Hürthle cell metamorphosis in the thyroid epithelium.

[The results of the intraportal allotransplantation of pancreatic islet cell cultures in patients with insulin-dependent diabetes mellitus].

The paper is concerned with the results of the treatment of 11 patients with insulin-dependent diabetes mellitus with surgical pathology by a method of intraportal allotransplantation of pancreatic islet cell (PIC) cultures and with similar surgical pathology without PIC transplantation. Carbohydrate metabolism and the level of plasma hormones were investigated. It has been shown that intraportal administration of PIC suspension into the liver can result in an earlier and more considerable decrease in a dose of exogenous insulin that is very important in the postoperative period.

[Clinical aspects of allotransplantation of the pancreatic islet cells in patients with diabetes mellitus and acute and chronic cholecystitis].

The results of treatment of 25 patients with acute and chronic cholecystitis accompanied by diabetes mellitus, who underwent transplantation of the pancreatic islet cells (PIC), and 40 patients with identical pathology, who underwent no PIC transplantation, are presented. In chronic cholecystitis and diabetes mellitus with the aim of preparation for the operation, PIC were administered intramuscularly, in acute cholecystitis and diabetes mellitus intraportally, after cholecystectomy. Transplantation of the PIC cultures in patients with diabetes mellitus contributes to correction of the impaired metabolic processes, reduction of the incidence of postoperative complications.

[Substantiation of indications for surgical and drug treatment of chronic diseases of the liver].

In the diagnosis of chronic liver diseases, the important place belongs to the evaluation of the immune status, indices of energetic metabolism. The preference should be given to the non-invasive instrumental methods. Drainage of the bile ducts is indicated in chronic active hepatitis and primary biliary liver cirrhosis.

[Malignization of goiter].

From analysis of literature data and the results of their personal studies embracing 3,139 operations for diseases of the thyroid gland the authors conclude that certain forms of goiter (nodular and recurrent in particular) play regularly the role of background processes in the development of cancer of the gland. This provides the ground for setting apart such patients into a risk group. Marked dysplasia is the main form of precancerous changes.

[Features of lipid peroxidation in patients with diabetes mellitus before and after therapy by a method of transplantation of human fetal islet cell cultures].

The authors discussed the results of changes in the intensity of lipid peroxidation and antioxidant activity in 70 patients with type I diabetes mellitus (severe forms) before and in 32 patients after the transplantation of human fetal pancreatic islet cell cultures. Lipid peroxidation, antioxidant activity, indices of glycolysis and carbohydrate metabolism were investigated. In some cases metabolic derangement was noted against a background of a decrease in the end products of lipid peroxidation but in a majority of diabetic patients an increase in LPO intensity was noted against a background of raised antioxidant activity, high activity of glycolytic reactions and inhibition of Krebs' cycle.

[Peptic ulcer recurrence after vagotomy].

The authors studied the clinical aspects, indices of gastric secretion and basal level blood serum insulin, gastrin and parathormone in 83 patients with ulcer disease recurrence after vagotomy. All patients showed an active acidopeptic factor--sequel of ineffective and incomplete (in some patients) vagotomy as well as inadequate pyloroplasty. Provoking factors of ulcer disease recurrence may be stress, drugs, duodenogastric reflux, presence of ligatures after pyloroplasty.
