Reaching a shared understanding of shared decision making in health care: NICE's experience of scoping the shared decision making guideline.

NICE's guideline on shared decision making, currently under development, endeavours to support shared decision making as part of routine health care practice. In this article, we summarize our learning to date, gained through the scoping of the guideline, on the key challenges that need to be addressed in the guideline. The production of a scope is the first stage in the development of a NICE guideline, setting the parameters for what will be considered in the guideline.

Fosaprepitant and aprepitant: an update of the evidence for their place in the prevention of chemotherapy-induced nausea and vomiting.

Introduction: The selective neurokinin-1 receptor antagonist aprepitant is effective in the treatment of acute and delayed chemotherapy-induced nausea and vomiting (CINV) associated with both moderately and highly emetogenic chemotherapy. Fosaprepitant has been developed as an intravenous prodrug of aprepitant.

Aims: To update the evidence underlying the use of fosaprepitant to prevent CINV.

Vildagliptin: the evidence for its place in the treatment of type 2 diabetes mellitus.

Introduction: Type 2 diabetes is increasing in prevalence worldwide and is a leading cause of morbidity and mortality, mainly due to the development of complications. Vildagliptin is an inhibitor of dipeptidyl peptidase 4 (DPP-4), a new class of oral antidiabetic agents.

Aims: To evaluate the role of vildagliptin in the management of type 2 diabetes.

Rimonabant: the evidence for its use in the treatment of obesity and the metabolic syndrome.

Introduction: Obesity and overweight affect over 1 billion people worldwide and are leading causes of morbidity and mortality. Clinical features of obesity converge with those of the metabolic syndrome and type 2 diabetes, greatly increasing the risk of long-term adverse outcomes.

Aims: To review the evidence on rimonabant, a novel CB1 receptor antagonist, for the treatment of obese and overweight patients.

Bevacizumab: the evidence for its clinical potential in the treatment of nonsmall cell lung cancer.

Introduction: Lung cancer is the leading cause of cancer-related mortality. Platinum-based chemotherapy is the usual first-line treatment for advanced nonsmall cell lung cancer (NSCLC), although an efficacy plateau has been reached with this approach. Bevacizumab is a recombinant, humanized, monoclonal antibody to vascular endothelial growth factor, which inhibits tumor angiogenesis and is being evaluated as a different mechanism to improve outcomes in patients with stage IIIB/stage IV (metastatic) NSCLC.

Introduction: Chemotherapy-induced nausea and vomiting (CINV) represents a significant burden on patients and healthcare systems. Despite the introduction of serotonin antagonists, many patients still experience CINV, particularly delayed symptoms occurring more than 24 hours after chemotherapy. Aprepitant is a selective neurokinin-1 (NK(1)) receptor antagonist approved for use with other antiemetics to prevent CINV caused by moderately to highly emetogenic chemotherapy.

Lumiracoxib: the evidence of its clinical impact on the treatment of osteoarthritis.

Introduction: The symptoms of osteoarthritis (OA) include joint pain, stiffness, and a reduced ability to perform normal daily activities, which result in decreased quality of life. There is currently no known cure or means of preventing the progression of joint damage due to OA. Therefore, treatment focuses on the control of symptoms, including the use of various agents [including nonselective and selective nonsteroidal antiinflammatory drugs (NSAIDs)] to provide pain relief and reduce inflammation.

Aripiprazole: the evidence of its therapeutic impact in schizophrenia.

Introduction: An ideal antipsychotic would rapidly stabilize acute psychotic symptoms and maintain the patient, without relapse, for prolonged periods in the absence of extrapyramidal, endocrine, diabetic, or cardiovascular side effects, and without weight gain. The dopamine partial agonist aripiprazole is compared with this ideal and with conventional antipsychotics, such as haloperidol, and with atypical antipsychotics.

Aims: To review the evidence for the clinical impact of aripiprazole in the treatment of patients with schizophrenia.

Introduction: Asthma, a respiratory disease associated with airway inflammation and hyperresponsiveness, is one of the most prevalent chronic diseases worldwide affecting both children and adults. Inhaled corticosteroids are considered to be the cornerstone of asthma management. Ciclesonide, an airway-activated inhaled corticosteroid, has been developed for the management of persistent asthma.
