Purpose: Low-velocity gunshot fractures (LVGFs) are a common type of gunshot-induced trauma with the potential for complications such as infection and osteomyelitis. The effectiveness of antibiotic therapy in LVGFs remains uncertain, leading to ongoing debate about the appropriate treatment. In this review, we evaluate recent updates on the current understanding of antibiotic therapy in LVGFs, how previous studies have investigated the use of antibiotics in LVGFs, and the current state of institutional policies and protocols for treating LVGFs with antibiotics.

Background: The incidence of periprosthetic femur fracture (PPFF) in the setting of total hip arthroplasty (THA) is steadily increasing. We seek to address whether there is a difference in outcomes between Vancouver B fracture types managed with ORIF when the original stem was a press-fit stem versus a cemented stem.

Methods: In this retrospective cohort study at a level 1 trauma center, we identified 136 patients over 65 years-of-age with Vancouver B-type fractures sustained between 2005 and 2019.

Using the multi-states (MuSt) theory as a framework, the present study focused on the individual (personality) antecedents of psychobiosocial states. Psychobiosocial states comprise emotional subjective experiences and their correlates (cognitive, motivational, volitional, bodily, motor-behavioural, operational, and communicative), that can be functional for performance (helpful) or dysfunctional (harmful). Specifically, we examined the relationships between two perfectionism dimensions (perfectionistic strivings and concerns) and functional and dysfunctional psychobiosocial states.

Article Synopsis
  • The COVID-19 pandemic led to the creation of a virtual orthopaedic rotation (VOR) to replace traditional visiting subinternships for medical students interested in orthopaedic surgery.
  • Three 3-week VOR sessions were conducted, featuring educational conferences, mentorship, and assessments of students' orthopaedic knowledge.
  • Results showed significant improvements in test scores, high satisfaction from participants, and a higher rate of interview invitations for VOR students compared to the overall applicant pool.

Mutations in the gene encoding the CDKL5 kinase are among the most common genetic causes of childhood epilepsy and can also give rise to the severe neurodevelopmental condition CDD (CDKL5 deficiency disorder). Despite its importance for human health, the phosphorylation targets and cellular roles of CDKL5 are poorly understood, especially in the cell nucleus. Here, we report that CDKL5 is recruited to sites of DNA damage in actively transcribed regions of the nucleus.

Objectives: The incidence of periprosthetic femur fracture in the setting of total hip arthroplasty is steadily increasing. Although the traditional dogma is that loose femoral components must be revised, we propose that in a frail geriatric population, anatomic reduction and fixation of Vancouver B2 and B3 periprosthetic fracture variants can restore stem stability and provide similar outcomes as revision arthroplasty.

Design: Retrospective cohort study.

Objective: Sports science has identified the trainer-athlete relationship in the etiology of injuries. We aimed to investigate: 1) the association between empowering (EMC) and disempowering (DMC) motivational-climate and musculoskeletal injuries in ballet, and 2) if EMC moderates the association between DMC and injuries.

Methods: A cross-sectional cohort survey-study was conducted online among ballet dancers (>18 yrs old) reporting acute and overuse injuries of the previous 2 years.

Article Synopsis
  • - Revision arthroplasty (RA) is seen as the gold standard for treating Vancouver type-B2 and B3 periprosthetic femoral fractures, while open reduction and internal fixation (ORIF) also shows good outcomes, sparking debate on the best approach.
  • - A systematic review shows no significant differences in clinical outcomes between ORIF and RA for Vancouver type-B2 fractures, but ORIF had higher rates of revision and reoperation for type-B3 fractures.
  • - Complications, mortality rates, and the ability to achieve full weight-bearing were similar between both surgical methods, though ORIF showed higher subsidence rates and variability in surgical techniques.

Introduction: Acetabular fractures in the elderly population are particularly challenging for orthopedic fracture surgeons to treat. Anterior column posterior hemitransverse (ACH) and both column (BC) fractures account for over 70% of these injuries in geriatric patients. Nonoperative management of these injuries has a mortality of about 79% and patients generally have a minimal chance of return to independent living.

Purpose: We propose that geriatric comminuted intra-articular distal humerus fractures can be effectively treated with a limited fixation approach aimed at achieving varus/valgus stability with columnar fixation, but allowing intra-articular comminution to heal by secondary congruency against an intact olecranon, thus avoiding an olecranon osteotomy.

Methods: Fifty-six elderly patients with AO 13-C type fractures, who underwent surgical fixation with ≥12-months of follow-up were retrospectively reviewed. Thirty patients were treated with intra-articular open reduction internal fixation (ORIF) with an olecranon osteotomy and 26 patients were treated with our limited fixation (L-ORIF) approach.

Based on Duda's conceptualization of the motivational climate, the Empowering and Disempowering Motivational Climate Questionnaire-Coach (EDMCQ-C) is a recently developed scale that assesses junior athletes' perception of the social environmental dimensions proposed by achievement goal theory and self-determination theory. The goal of the current investigation was to evaluate the Lithuanian translation of the EDMCQ-C and more broadly extend the validity and reliability of this questionnaire in sport participants. 712 adolescents from different sport teams in Lithuanian were the participants in this study.

Introduction: Implant cut-out remains a common cause of cephalomedullary nail (CMN) failure and patient morbidity following surgical treatment of intertrochanteric femur fractures. Recent studies have suggested an increased rate of CMN cut-out with helical blades as opposed to lag screws. We compared rates of overall cut-out between helical blades and lag screws and used bivariate and multivariate analysis to determine the role of proximal fixation method among other variables on risk for cut-out.
