We reported a case of 27-year-old woman who suffered a hydrothorax induced by a central venous catheter that had been placed to facilitate parenteral nutrition. The central venous catheter was inserted into the superior vena cava through the right subclavian vein. Chest radiograph after insertion revealed proper position of the tip.

Background And Aims: Since 1999, the Dutch trauma care has been regionalized into ten trauma systems. This study is the first to review such a trauma system. The aim was to examine the sensitivity of prehospital triage criteria [triage revised trauma score (T-RTS)] in identifying major trauma patients and to evaluate the current level of trauma care of a regionalized Dutch trauma system for major trauma patients.

Objective: The aim of this study was to describe perioperative morbidity and mortality of patients presenting with resectable lung cancer and to investigate the long-term survival.

Methods: We reviewed the records of 344 patients who underwent lung resection for bronchogenic carcinoma. Follow-up information was obtained from visits to the outpatient clinic.

Background: Despite significant advancements in rectal surgery, poor perineal wound healing after abdominoperineal resection (APR) of the anorectum continues to be a potential complication of the procedure. The aim of this prospective randomized multicenter study was to investigate the efficacy of a new mode of local antibiotic administration.

Patients And Methods: Ninety-seven patients who had to undergo APR for low rectal carcinoma either received sacral drainage plus primary wound closure (control group, n = 48) or the same treatment and supplementary application of three resorbable gentamicin-impregnated collagen fleeces (Septocoll; Merck Biomaterial GmbH, Darmstadt, Germany; Genta group, 49).

Background: Several studies suggest that perioperative blood transfusion is a major independent risk factor for postoperative bacterial infections. Transfusion-induced immunosuppression is thought to mediate this effect.

Study Design And Methods: In a randomized clinical trial comprising 697 patients with colorectal cancer, the relationship between two types of red cell components (buffy coat-depleted packed red cells and white cell-reduced [filtered] packed red cells) and postoperative bacterial infections was analyzed.

In retrospective studies, perioperative blood transfusions were associated with poor prognosis after surgery for cancer and were a major independent risk factor for postoperative bacterial infection. Leucocyte-depleted, in contrast to buffy-coat-depleted, blood has no immunosuppressive effects in transplantation and so might lack detrimental effects on cancer prognosis and postoperative infections. We studied this hypothesis in a controlled trial by randomly allocating patients to receive either leucocyte-depleted red cells or packed cells without buffy coat when blood was needed.

The incidence of nerve injuries in primary shoulder dislocation and humeral neck fracture is uncertain. We made a prospective study of 101 patients, using clinical examination and extensive electrophysiological assessment when there was suspicion of nerve damage. We found electrophysiological evidence of nerve injury in 45%, most involving the axillary, suprascapular, radial and musculocutaneous nerves.

The case is reported of a female aged 32 years in whom, after laparoscopic cholecystectomy performed because of symptomatic gallbladder calculi, leakage from the cystic duct occurred. After percutaneous drainage of the perihepatic bile collection, endoscopic retrograde cholangiopancreatography (ERCP) with papillotomy was performed; since this proved insufficient, an endoprosthesis was inserted into the common bile duct by ERCP, following which the bile leakage ceased.

Interlocking nails have expanded the indication for intramedullary fixation to almost any type of fracture of the shaft of the femur. Distal locking with the Brooker-Wills femoral nail is obtained by a wing fixator. This simplifies the operative technique and diminishes exposure to radiation.
