Background A growing literature examines the way two changes in primary care - the shift towards remote working, and the diversification of practice teams to incorporate, for instance, physician associates and paramedics - affect patient care within the practice. However, little is known about these changes' effects on community nurses. Aim To explore community nurses' experiences of delivering palliative care in the context of GPs' new ways of working.

With the expansion of wireless mobile networks into both the daily lives of individuals as well as into the widely developing market of connected devices, communication is an increasingly attractive target for attackers. As the complexity of mobile cellular systems grows and the respective countermeasures are implemented to secure data transmissions, the attacks have become increasingly sophisticated on the one hand, but at the same time the system complexity can open up expanded opportunities for security and privacy breaches. After an in-depth summary of possible entry points to attacks to mobile networks, this paper first briefly reviews the basic principles of the physical layer implementation of 4G/5G systems, then gives an overview of possible attacks from a physical layer perspective.

Vitamin A resolves lineage plasticity to orchestrate stem cell lineage choices.

Lineage plasticity-a state of dual fate expression-is required to release stem cells from their niche constraints and redirect them to tissue compartments where they are most needed. In this work, we found that without resolving lineage plasticity, skin stem cells cannot effectively generate each lineage in vitro nor regrow hair and repair wounded epidermis in vivo. A small-molecule screen unearthed retinoic acid as a critical regulator.

Type 2 innate lymphoid cells are not involved in mouse bladder tumor development.

Therapies for bladder cancer patients are limited by side effects and failures, highlighting the need for novel targets to improve disease management. Given the emerging evidence highlighting the key role of innate lymphoid cell subsets, especially type 2 innate lymphoid cells (ILC2s), in shaping the tumor microenvironment and immune responses, we investigated the contribution of ILC2s in bladder tumor development. Using the orthotopic murine MB49 bladder tumor model, we found a strong enrichment of ILC2s in the bladder under steady-state conditions, comparable to that in the lung.

Intravesical Ty21a treatment of non-muscle invasive bladder cancer induces immune responses that correlate with safety and may be associated to therapy potential.

Background: Standard of care treatment of non-muscle invasive bladder cancer (NMIBC) with intravesical Bacillus Calmette Guérin (BCG) is associated with side effects, disease recurrence/progression and supply shortages. We recently showed in a phase I trial (NCT03421236) that intravesical instillation in patients with NMIBC with the maximal tolerated dose of Ty21a/Vivotif, the oral vaccine against typhoid fever, might have a better safety profile. In the present report, we assessed the immunogenicity of intravesical Ty21a in patients of the clinical trial that had received the maximal tolerated dose and compared it with data obtained in patients that had received standard BCG.

Hybrid Analog Computer for Modeling Nonlinear Dynamical Systems: The Complete Cookbook.

This paper describes a design process for a universal development kit based on an analog computer concept that can model the dynamics of an arbitrarily complex dynamical system up to the fourth order. The constructed development kit contains digital blocks and associated analog-to-digital and digital-to-analog converters (ADCs and DAC), such that multiple-segmented piecewise-linear input-output characteristics can be used for the synthesis of the prescribed mathematical model. Polynomial input-output curves can be implemented easily by four-quadrant analog multipliers.

Tumor-Microenvironment Characterization of the MB49 Non-Muscle-Invasive Bladder-Cancer Orthotopic Model towards New Therapeutic Strategies.

Bacillus Calmette-Guérin (BCG) instillations for the treatment of non-muscle-invasive bladder cancer patients can result in significant side effects and treatment failure. Immune checkpoint blockade and/or decreasing tumor-infiltrating myeloid suppressor cells may be alternative or complementary treatments. Here, we have characterized immune cell infiltration and chemoattractant molecules in mouse orthotopic MB49 bladder tumors.

Ras drives malignancy through stem cell crosstalk with the microenvironment.

Squamous cell carcinomas are triggered by marked elevation of RAS-MAPK signalling and progression from benign papilloma to invasive malignancy. At tumour-stromal interfaces, a subset of tumour-initiating progenitors, the cancer stem cells, obtain increased resistance to chemotherapy and immunotherapy along this pathway. The distribution and changes in cancer stem cells during progression from a benign state to invasive squamous cell carcinoma remain unclear.

Electronic Tunability and Cancellation of Serial Losses in Wire Coils.

This work presents a novel methodology to adjust the inductance of real coils (electronically) and to cancel out serial losses (up to tens or even hundreds of Ohms in practice) electronically. This is important in various fields of electromagnetic sensors (inductive sensors), energy harvesting, measurement and especially in the instrumentation of various devices. State-of-the-art methods do not solve the problem of cancellation of real serial resistance, which is the most important parasitic feature in low- and middle-frequency bands.

Community-based anticipatory prescribing during COVID-19: a qualitative study.

Methods: Semistructured qualitative interviews were conducted with a purposive sample of 16 healthcare professionals involved in community palliative care. Data were analysed inductively using thematic analysis.
