Article Synopsis
  • - Transitional care programs (TCPs) involve hospital staff following up with patients after discharge to reduce the chances of returning to the hospital or emergency department and to lower healthcare costs.
  • - A study analyzing patient data from Greenville Memorial Hospital showed that patients who participated in TCP had significantly fewer hospital readmissions and lower costs compared to those who did not participate.
  • - While TCP graduates had reduced readmission rates and overall costs, there was no significant difference in emergency department visits after 30 and 90 days, suggesting the need for further research to verify these effects.
